• Who was the relationship with?

• How long did it last and when did it finish?

• What was the status of the relationship?

• Why did the relationship end?

• Were there any children from the relationship?

• With whom did they live after the relationship ended and how was this decided?

• How were the children affected by the breakup of this relationship?

• How did you help them deal with this?

• What contact is there now with the ex-partner and any children?

• If contact for children is, or was involved, how has this been managed?

• If children from a previous relationship visit or stay over, is there sufficient physical space in the home?

• What is the attitude of these children to this fostering application?

• How will their needs be met, alongside those of the foster children, when they visit?

• What impact might these relationships have no fostering?

• What have you learnt from your relationship/s?

• Can we contact the ex-partner for a reference?

• If not, what are the reasons for this?

• If this is not possible, are there referees who knew you when you were in that relationship who could be contacted?

• If the ex-partner has not responded, detail what efforts were made to try to obtain this reference.

Other significant relationships

• Who was the relationship with?

• At what stage of your life was this at?

• What was the nature of the relationship?

• What effect has the relationship had on you?

• Is the relationship still ongoing?

• If it has ended, how did it end and what was the effect of this on you?

Assessor’s analysis

• Is there corroboration of the details of any previous relationship from the ex-partner or a referee who knew the applicant and the ex-partner when they were together?

• If there are children from a previous relationship, what is the evidence that their needs have been fully considered in this application?

• Can any experience of arranging contact for their own children be helpful in terms of thinking about contact for foster children?

• What evidence is there that the current relationship (if there is one) is more likely to succeed that then the previous one?

• Are there any potential areas of vulnerability for the applicant in terms of fostering?

• Are there any issues arising from any other significant relationships the applicant has had that might have a bearing on this application?
