MND Victoria Equipment ServiceThe MND Victoria Equipment Service (ES) provides a limited range of commonly used equipment and disability aids for people living with MND who are registered with MND Victoria and living in the community within Victoria. The equipment is standard in size and where possible has options for flexibility e.g. adjustable height. The needs of people requiring purpose built or modified equipment are generally met through other sources such as the Aids and Equipment Program (A&EP) administered by the Victorian Department of Human Services.The ES operates on a re-issue model and items are loaned free of charge to MND Victoria clients, including no cost delivery and retrieval. MND Victoria intends to pay once only for the delivery and/or retrieval costs of an item. The client may be required to pay for subsequent deliveries of the same type of equipment if they change their mind about wanting the item. EligibilityClients must be registered with MND Victoria and be a Victorian resident living at home in the community. Clients need to provide to MND Victoria a written confirmation of their diagnosis from their Doctor or Neurologist. They must also sign the MND Victoria Equipment Service Loan Agreement agreeing to its terms and conditions. Equipment itemsThe ES Allied Health Professional (AHP) Stock List can be accessed through the MND Victoria website at mnd.asn.au and is found under Get Support -> Equipment Service. It provides basic information about the items available through the ES. Certain low cost items are provided on a non-retrievable basis. Short term interim hire may be available for selected items when nominated as urgent by the requesting AHP however items are not available to trial.4.Submitting a request AHP Equipment Request forms must be completed by the requesting AHP and emailed to equipment@mnd.asn.au. They are accessed through the MND Victoria website at mnd.asn.au under Get Support -> Equipment Service or also under Health Professionals. 5.Processing AHP Equipment Request Forms Receipt of the emailed request will be acknowledged by the form being returned to the requesting AHP by email within 10 working days of receipt. The returned request will include a registration number and a date of processing. A copy of the request will be sent to the relevant MND Victoria Regional Advisor (RA). Your client will also receive acknowledgement in writing of the request.6.Prioritisation and allocation of equipment requestsEquipment requests are prioritised according to urgency, determined on advice from the requesting AHP and MND Advisor Prioritisation takes place at the time of the request being registered. Special note is made if the request is deemed urgent by the requesting AHP or RA. Priority status can be revised and updated as needs change on request from the referring AHP and/or members of the Support Services (SS) team. Allocation is made by Coordinator Equipment Service (CES) and/or delegate on an ongoing basis according to agreed priority and availability of equipment.7.Equipment hire The following three items, deemed essential for quality of life, will be hired by MND Victoria when the AHP has identified an urgent need and MND Victoria equipment is unavailable:standard attendant propelled shower/toilet commode,walking frame, manual wheelchair,The item will be replaced by MND Victoria, when an ES item becomes available. Where ever practicable hired equipment will be the equivalent make and model to the MND Victoria item. Hired equipment will only be replaced by standard ES equipment. If the client is not satisfied with the replacement equipment provided by MND Victoria, they will need to make private arrangements.8.Delivery advice The requesting AHP and the relevant MND Victoria RA will be advised, by email that the equipment has been allocated and requested for dispatch.9.Instruction in use of equipmentInstruction in the use of the equipment is the responsibility of the requesting AHP. It is expected that this will be arranged at the time the equipment is prescribed and completed at the time of delivery or as soon after as practicable. Written instructions (when available) are provided by MND Victoria with equipment at delivery, however these are for information purposes only and do not relieve the AHP from their responsibility. Where the requesting AHP has arranged for a local AHP to provide training it is helpful to provide contact details on the request form so all associated AHP’s can be advised when despatch is arranged. 10.Modifications Modifications to MND Victoria equipment items are discouraged however may be considered in exceptional circumstances. Applications for the modification of equipment must be made in writing by the prescribing AHP to the Manager Support Services (MSS). Approval will be at the discretion of MSS after consultation with CES, CSS and RA. All costs of modification and restoration of equipment must be met by the client.11.Clients moving to aged care servicesClients moving into a Commonwealth Government funded Residential Aged Care Service (RACS) may take the equipment they currently have on loan with them, with the exception of beds, hoists and pressure care. The RACS, as per the A&EP Guidelines, is responsible for providing any further equipment the person requires for use in the residential facility. The RACS will be required to sign the RACS Equipment Agreement form agreeing to its terms and conditions. In instances where the residential facility, after exhausting all available options, is unable to source the required equipment, requests for certain MND Victoria equipment items may be considered according to need and availability. Even when all of the above criteria are met, approval is entirely at the discretion of the MFSS and/or MND Victoria CEO and is determined on an individual basis. 12.EnquiriesThe CES or delegate is available for telephone enquiries regarding equipment Monday – Friday, 9.00am – 5.00pm, or by email on dharkin@mnd.asn.au or equipment@mnd.asn.au.
