Instructions for Staff & Patients to retrieve COVID-19 Vaccination Information from the MyChart Patient Portal

1. Log into the MyChart Patient Portal 2. Click "Menu" 3. Under the "My Record" heading, select "COVID-19"

4. Select the down arrow next to the vaccination to retrieve the details 5. To print the information, click the "Print" button in the upper right corner. Save the file as a pdf

to a location of your choice.

UPLOADING YOUR COVID-19 VACCINATION RESULTS THROUGH WOLVERINE ACCESS

1. Access Wolverine Access (Wolverine Access - Find & Favorite University of Michigan Resources (umich.edu)

2. Select the COVID-19 Tile or type COVID-19 in the search option to find the COVID-19 tile

3. Once the COVID-19 tile has been selected, select Report COVID-19 Vaccination Status.

4. Follow the instructions on the Report COVID-19 Vaccination Status page. Please make sure that the first line states Greetings "Your Name" to ensure you are appropriately logged into Wolverine Access.

5. When you arrive to the question asking to upload a copy of your COVID-19 vaccination card, utilize your COVID-19 vaccination card, or, utilize the pdf file you created by accessing the MyChart Patient Portal (instructions above).

6. If you have received the COVID-19 vaccination and DO NOT have a copy of your COVID-19 vaccination card, OR, you DO NOT have an active MyChart Patient Portal to access your COVID19 results, please contact OHS at 723-764-8021
