The John Glenn Chapter of National Honor Society

187452021399500The Eisenhower Chapter of National Honor SocietyCongratulations on choosing to apply for membership in the National Honor Society. As you complete the application, please keep the following in mind:For many students, selection as a member of the National Honor Society is the pinnacle of their achievements in school. This honor, recognized throughout the nation, is both the public recognition of accomplishment and the private commitment to continued excellence on the part of the new member. The selection to NHS is a privilege, not a right. Students do not apply for membership in the National Honor Society; instead they provide information to be used by the local selection committee to support their candidacy for membership. Membership is granted only to those students selected by the Faculty Council in each school. This is not an election, nor is membership automatically conveyed simply because a student has achieved a specified level of academic performance. NHS is more than just an honor roll; leadership, character, and service also play a major part. (NHS Handbook, 15th edition.)Directions: Please complete all sections. Do not be modest. Every bit of information can be used by the Faculty Council to assist with the selection process. Completion of this application does not guarantee selection. Please return completed application to Mrs. Sides in room A108 or Mr. Wieland in room A211 by 3:00pm on Friday, March 6, 2020.Verification Form: Attach the verification form on the top of all of the other information. This will be the only place your name appears. The faculty council will not see this page, but it must be completed in full for your application to be submitted.Sections I – V: Please type the requested information directly into Microsoft Word. There should be no handwriting in these sections. ONCE SECTIONS I – V ARE COMPLETED, PLEASE PRINT THE APPLICATION—SINGLE-SIDED ON REGULAR PRINTER PAPER—SO THAT YOU CAN COMPLETE SECTION VI.Essays A – C: Complete each essay in your own handwriting with 200 words or less. Please use a blue or black pen. Be specific to help the panel better understand you. Essays should not be written on lined notebook paper; they must be written in your own handwriting on the pages of the application.Recommendation Forms: Find three adults unrelated to you who are willing to complete an online recommendation form to support your bid into the National Honor Society. Exactly one recommendation must be completed by a Mill Valley teacher who has been witness to your character and leadership within the classroom, one must be completed by someone who has witnessed those same qualities outside the classroom (it could be a coach—school-sponsored or otherwise, sponsor, boss, church leader, etc.), and the final recommendation must be completed by your seminar teacher. The final page of this document must be cut in thirds and given to those individuals who have agreed to complete your recommendations. All recommendations must be submitted online by the application deadline of Friday, March 6. Reminders: Except for the verification form, your name should not appear anywhere on the application. Please paper clip all materials. Do not staple or place the application materials in a folder or binder.VERIFICATION FORMcenter136118I understand that completing this form does not guarantee selection to NHS, and that the following information presented here is complete and accurate.5387220102235circle onecircle onePrint Student Name: ____________________________________________Current Grade: 10th or 11th ______________________________________________________________________Student SignatureDateI have read the information provided by my son / daughter on this form and can verify that it is true, accurate, and complete in its presentation.______________________________________________________________________Parent SignatureDateI.
