The following plan will be implemented and followed at our facility all times. SAFETY PLAN I. PEOPLE Physical Distancing – We are committed to ensure employees comply with physical distancing requirements by: Limiting capacity within the health club to no more than 33% of our maximum occupancy for areas set by our certificate of occupancy at any given time, inclusive of employees and patrons. This will be mandated through our check-in process at front desk. Each member and employee must check in with their key-tag upon arrival and after health screening. Members are asked to complete their workout within one hour.The total occupancy at Jada Blitz at 33% is 145 patrons. Each room will be monitored as well by staff for individual room occupancy. All patrons must wear an acceptable face covering at all timesOur facility is clearly marked upon arrival that face masks are required Correspondence has also been sent out to all members communicating thisAll patrons must pass a health screening that we are administering digitally before their entrance into the gym. We are using This meets the requirements under Section IIIAll patrons will be required to sign -in via their membership key tag which provides their name, address and phone number. Both check-in information and health screening will be maintain via digital records for more than 28 days. All patrons will be temperature screened after scanning their key tag. All patrons must maintain social distancing of at least 6 feet. We have moved or marked all equipment to meet these requirements. There are social distancing stickers and reminders throughout the facility. Jada Blitz will ensure that all individuals, including employees and patrons, will wear an acceptable face covering. Jada Blitz does have a café Jada Blitz Is aware of acceptable face coverings and has communicated such. Jada Blitz will also have on hand disposable approved masks to provide to patrons and employees. Jada Blitz has modified the use of workout equipment and stations so that all individuals are at least 6 feet apart in all directions. Jada Blitz will have floor monitors to ensure that patrons are cleaning their equipment after use. There are also stickers and signage everywhere to remind patrons of this. Jada Blitz has littered the facility with signage to remind of the DOH COVID 19 points. Gathering in Enclosed SpacesJada Blitz has committed to limiting in-person employee gatherings (staff meetings, break rooms, etc.) to the greatest extent possible. Adequate social distancing practices have been put in place for smaller areas such as locker rooms, restrooms, and breakrooms. Locker Rooms – Jada Blitz has developed a token system for locker use. All available lockers are spaced out 6 feet. All lockers that are available have a magnet on front. After use the token is brough back up front and the maintenance staff clean and disinfects before that locker can be used again. Bathrooms – each bathroom is clearly marked that no more than two stalls can be used at a time. Employee Breakroom – no more than 3 employees can be in breakroom at a given time. Jada Blitz has individual showers and will reopen those after Labor Day. We will use a token system like the locker rooms upon reopening. Jada Blitz employee schedules are staggered where possible and no more than 3 people in breakroom at a time. On-Site Activity Jada has coordinated with local DOH for inspection prior to opening. Jada requires appointments and reservations for group classes, personal training sessions, consultations, and tours. Jada Blitz will keep all group fitness classes outdoors until the weather no longer permits. All classes must be booked via our Wellness App While classes are outside, we will ensure that the number of individuals permitted by the State’s local gathering limit are not exceeded. When classes are moved in-doors they will not exceed 33% of the typical class size Classes will be conducted so that all individuals have a minimum of 6 feet of space Jada will ensure that all individuals, including the instructors and patrons, maintain at least 6 feet of distanc3e between one another and that facemasks are worn.Jada will ensure that all equipment is cleaned and disinfected between use.Wipes are provided outside for classes and in every room in the facility Post class the Clorox Total 360 machine will also be used. Jada will encourage patrons to bring their own yoga mats for yoga and Pilate classes Jada Blitz does perform personal training services. Trainers will ensure their clients wear their face coverings at all times. Trainers will ensure that they maintain 6 feet of space to the maximum extent possible. Trainers will ensure they prohibit the sharing of personal items between one another All trainers are provided their own bucket of disinfecting wipes to use through their sessions and after equipment is used. Jada Blitz will be prohibiting high-risk activities where physical contact cannot be continuously avoided- ie: boxing Jada Blitz will be closing the following in the facility: SaunasLounge Areas in Reception Area Water fountains (water bottle refill stations are still open) Jada Blitz is taking measures to reduce interpersonal contact by: Limiting our hours Conducting all group classes outdoors Reminding patrons that their workouts are limited to 1 hour All front-end operational employees are assigned to a station for their shift and they have their own phone, computer, and register. It will be disinfected throughout their shit and at the end. All equipment that cannot be assigned needs to be disinfected after use. Jada Blitz has reduced the operational hours by 3 hours each day – we also will be closed each day from 1pm – 2pm for deep cleaning. Movement and Commerce Jada Blitz will monitor and control the flow of traffic into the gym to Ensure maximum capacity and social distancing requirements are met That all individuals have signed in and completed the health screening There will be an employee outside directing and monitoring this at all times. Jada Blitz has put in place bi-directional foot traffic using tape and arrows at the entrance and through the facility. Social distancing stickers have been placed outside and throughout the facility. The entrance doors are designated and marked for entrance and exit Stickers and tape are used outside to maintain cues for social distancing. Jada will ensure that all staff at the shake bar and retail area perform hand hygiene before and after transferring items. Jada Blitz has a touchless check-in process and we also store patron’s payment information to make payment options touchless as well. II.PLACESAir Handling Systems Jada Blitz’s facility has central air handling systems. Jada Blitz has ensured that our HVAC system filtration meets the highest rated filtration compatible with our system. Jada Blitz has installed MERV-13 filtration in all HVAC units. Jada Blitz has certification by a HVAC technician of this. Protective Equipment Jada Blitz is committed to ensure that employees and patrons are only permitted entry into the gym with an acceptable face covering. Jada Blitz is committed to ensure that all individuals wear their face coverings at all times while in the facilityOnly exceptions is to remove face covering to eat or drink – but must maintain 6 feet in doing so. Jada Blitz has secured and stocked acceptable face coverings to supply employees and patrons with should they not have theirs. All Jada employees have been advised that they cannot share face coverings and that all face coverings worn must be cleaned or replaced after use. Jada Blitz has designed a system to limit the sharing of objects by assigning stations for front end employees. All workers are required to wash hands and sanitize shared surfaces. Hygiene, Cleaning, and Disinfection Jada Blitz is in adherence to the hygiene and cleaning and disinfection requirements as advised by the CDC and DOH. We do and will continue to maintain cleaning, disinfection logs that include the date, time and scope of cleaning and disinfection around the facility. Jada Blitz has provided and will maintain hand hygiene stations on site: Hand sinks with warm water, soap and paper towels: Men and women’s locker rooms Shake Bar Balanced Body Café Hand sanitizing stations throughout the facility that contain an alcohol-based sanitizer containing at least 60% alcohol. The hand sanitizer stations are available throughout the gym for use of both employees and patrons. Jada Blitz has placed signage near hand sanitizer stati0ons indicating that visibly soiled hands should be washed with soap and water. Jada Blitz has placed garbage receptables around the gym to discard soiled items. Jada Blitz is providing free towel service and our maintenance staff has read and signed off on the best practices of laundering the towels. Jada Blitz will conduct regular cleaning and disinfecting of the gym and more frequent cleaning and disinfection for high risk areas and highly touched surfaces. This will be done throughout the course of operating and be performed rigorously by our maintenance staff and employees. Floor monitors and signage to remind people to wipe down equipment after use. Frequent wiping down by maintenance staff and use of Clorox Total 360 machine Appropriate wipes are stocked throughout the facility for patrons to utilize and personal trainers will have their own bucket Jada Blitz will ensure that regular cleaning and disinfecting of locker rooms and restrooms occurs throughout the day. Token system for lockers will tell maintenance staff when lockers need to be cleaned Cleaning logs in bathroom as well and will be cleaned at least every two hours Reduced capacity in bathrooms with reminders via signage Jada Blitz will ensure that equipemnt is regularly cleaned using disinfectants that are registered:Clorox Total 360 MachineNeutral EQTrushot Disinfectant Lysol Disinfectant Wipes Jada Blitz is committed to the cleaning and disinfection of an exposed area in the event that an individual is confirmed to have a positive case of COVID. Jada Blitz is committed to following the CDC guidelines on “Cleaning and Disinfecting Your Facility” if someone is suspected or confirmed to have COVID. Jada Blitz has communicated and prohibited sharing food and beverages amongst employees. Phased Reopening Jada Blitz will phase in re-opening activities such as in-door classes. Jada Blitz has also limited operational hours Communications PlanJada Blitz has affirmed that we reviewed and understand the state-issued industry guidelines and will implement the same. We have sent out communication to all employees and required sign-off onWe have encouraged everyone to adhere to the CDC and DOH guidance Signage has been posted inside and out to remind of proper hygiene, social distancing rules, PPE, and cleaning protocols. III.Processes Screening and Testing Jada Blitz will be implementing mandatory daily health screening practices of employees and patrons. We are utilizing a digital screening site that will allow both employees and patrons to take the health screening prior to entering our facility. Screening before entering will prevent individuals from forming a line at the front desk and being in close proximity of one another. The questions or our screening are: Covid Symptoms – have you tested positive for COVID-19 in the past 14 days or experienced symptoms of COVID-19 in the past 14 days? Travel – Have you traveled within the past 14 days to one of the states designated to have significant community spread of COVID-19 and spent more than 24 hours in that state? Have you knowingly been in close contact or proximate contact in the past 14 days with someone who has tested positive for COVID-19 or who has had symptoms of COVID-19? Screening practices for temperature checking will include: Adequate social distancing while in line for building entry Only allowing individuals who have been screened to be admitted past the front deskJada Blitz will ask employees who answer any of the screening questions positively to disclose important facts relative to those questions. In addition, temperature screening will be conducted per the DOH guidelines. All staff who will be conducting temperature checking has been trained how to do so to protect themselves from exposure to infectious individuals. Screeners will be provided face masks and also have access to gloves Any individual who screens positive for COVID symptoms must not be allowed to enter the facility and will be sent homeEmployees will be provided with information on healthcare and testing resources Jada Blitz will immediately notify the state and local health department about the case if test results are positive Jada Blitz has designated our Operations Director to be responsible for attesting to having reviewed al questionnaires. Jada Blitz has designated owner of the facility to be a point of contact for the gym as a site safety monitor and will be responsible for continuous compliance with all aspects of the site safety plan. Jada Blitz will maintain a log of every person, including employees and patrons and will include contact information, including: full name, address, and phone number. This is all tracked in our membership data software – ABC financial – when a patron and employee arrive to facility and scan their key tag This information will be available for 28 days Tracing and Tracking Jada Blitz will notify the state and local health department immediately upon being informed of any positive COVID-19 test resulting by an individual at their gym or fitness center. In the case that someone tests positive, Jada Blitz must cooperate with the state and local health department as required to trace all contacts in the gym or fitness center and notify local and state of the individuals entering the gym dating back 48 hours before the individual first experienced COVID-19 symptoms or tested positive. Confidentiality will be maintained as required by federal and state law regulationIf an individual was to show symptoms while in the gym or fitness center, Jada Blitz will notify employees in the surrounding areas. Employees who are alerted that they have come in close contact with a person with COVID-19, and have been altered via tracing, tracking or other mechanism, are required to self-report to Jada Blitz. IV. Employers Plans Jada Blitz will have our safety plan on site for employees and have required all employees to review. ................
