CHANGE SUMMARY:

After-hours Services Information for Collection Sites - CDL PORTION OF CONTRACT ONLY

EMSI will be utilized for mobile collection services to come on-site to conduct specimen collections as needed.

Site Selection Information: No matter what kind of test needs to be done (pre-employment, random, reasonable suspicion, post-accident, return-to-duty or follow-up), you must check the listed collection hours for the collection site(s) in your county. You may only send employees to collection sites within their listed normal business hours. EMSI is the only collection entity that can be used for any services that occur outside of those normal business hours (which are referred to as after-hours)

For any after-hours situation that would be considered an "emergency", which would be a reasonable suspicion or a postaccident test ? you must call the toll-free EMSI number (800-421-3674), and they will make arrangements to come to the location and collect the specimen. The EMSI collector will arrive with all materials needed for the collection, including the materials (bluing for the toilet water, tape to restrict faucets, etc.) needed to restrict the employee's access to water within the bathroom.

Any after-hours situation that would be considered "routine", which would be random, return-to-duty or follow-up, must be

pre-scheduled 72 hours in advance. For these collections, you must call one of the three regional centers (Harrisburg, Philadelphia or Pittsburgh ? whichever is closest) to pre-schedule the collection, and they will make arrangements for the collector to come to the location and collect the specimen. Please use the phone number associated with the EMSI regional center that you have chosen.

When you call EMSI, you must provide them with your company/account name and your EMSI account number. Each agency has their own account name and number.

CHANGE SUMMARY:

Phamatech has agreed to provide all using agencies with signed and dated positive reports via fax if requested. Fax numbers must be provided to vendor and reporting will occur according to agency requirements.

A revised Collection Site list is attached for using agencies that are covered by the contract where collection sites are required.
