PROVIDER INFORMATION
EFFECTIVE JANUARY 1, 2018
The district has chosen Texas Mutual as our workers' comp carrier with uses the Workwell Network to provide health care to employees who are injured at work.
Employees are expected to follow the procedures below if injured at work.
? It is required by Texas Mutual that all employees sign an Employee Acknowledgement of Workers' Compensation Network form after each incident/accident indicating that employees are notified about using a Workwell Network healthcare provider.
? The first report of injury/illness (DWC-1) form must be completed and submitted to the Human Resources Services office via fax at 281-412-1303 as soon as possible. This form can be found on the district's website under benefits at: .
? If an employee is injured on the job and it is a life threatening emergency do not hesitate to call "911" for emergency assistance or go to the nearest emergency room. However, if the injury is not life threatening, the Workwell Network requires that employees choose a treating doctor that is listed in their network.
? After initial emergency care, select a network doctor from the Workwell Network () for any ongoing care necessary.
? To find a network provider call (844) 867-2338 or go to ? health care network ? find a network doctor.
? If an employee chooses to select a treating doctor that IS NOT listed on the Workwell Network's approved list of treating doctors, they will be responsible for any medical bills that result from seeking treatment from an out-of-network doctor.
