Mental Health Week 2015Small Grant Application KitThis year in the Barwon region our objective for Mental Health Week is to encourage connectedness on all levels: between our mental and physical health, with our community, between organisations and groups and with mental health drugs and alcohol services. ACT BELONG COMMIT is the theme for Mental Health Week:ACT to ConnectConnect to BELONGCOMMIT to Connect Mental Health Week will be celebrated from Sunday, 4 October to Saturday, 10 October 2015. World Mental Health Day is marked every year on 10 October.Barwon Health is once again offering Wellbeing Grants of up to $300 to assist individuals, organisations, and community groups in our region to run activities either during or commencing in Mental Health Week. Activities need to demonstrate positive health and wellbeing benefits. Some of the events previously funded include: relaxation, yoga, music, mindfulness, and professional development sessions.We invite you to complete the following application with a view to supporting activities that promote connectedness and positive mental health. I have completed the small grants application form 2.If we are successful in receiving a small grant, we are prepared to: hold or commence our activities during Mental Health Week 2015, acknowledge Mental Health Week associated with the activities by using the logo and advise Barwon Health Mental Health if the activities cannot proceed for any reason3.I will submit a tax Invoice to Mental Health Week Small Grants, Mental Health Drugs and Alcohol Service, Barwon Health along with all receipts associated with the activities along and the organisations ABN. 4.By making this application you agree that if you are successful, but are for some reason not able to run your activities, you will return the grant funds to Barwon Health Mental Health by 31st October 20155.I understand my application must be emailed to Leo Coolhaas (leocool@.au) by 5.00 p.m. on 16th September 2015.I understand I cannot mail or fax my applicationI agree to accept engagement with staff from the Mental Health Drugs and Alcohol Service at some stage during the promotion, commencement or delivery of the activity.I agree to complete an evaluation sheet for submission to Mental Health Drugs and Alcohol Service for collation into the final report.It will be important that your organisation considers your own publicity opportunities for events which are open to the general public.
