HOUSING PROGRAMS AND SERVICES: DESCRIPTIONS AND CONTACT INFORMATION

Current Housing Programs - The Mental Health Association in Tulsa, Inc. (Association), has been actively involved in developing and operating a continuum of housing and support services for persons with serious mental illnesses since 1989. Currently, the Association owns 256 housing units and leases 31 units in a variety of types and locations. Most of the current housing is available to homeless or formerly homeless adult individuals, however beginning in 1999 the Association began developing housing that is not limited to homeless persons with a focus on community reintegration through use of the mixed income model.

The following housing programs and services are currently available through the Association:

1. Homeless Outreach (Outreach and Case Management): Through a grant from HUD, the Association contracts with Family and Children’s Services to provide street and shelter outreach, case management, nursing and a psychiatric clinic for homeless persons with serious mental illnesses. This team feeds into our entry level housing by functioning as the gatekeeper for the Safe Haven at the YMCA. Contact: Homeless Outreach Team for F&CS: 582-1200

2. Safe Haven @ “Y” (25 single room occupancy units): 100% for street habituated homeless adult individuals with mental illness who agree to enter into a transitional or permanent living program that has a “low demand/high expectation” philosophy of care. No maximum stay but there is an expectation of the individual achieving the skills, resources and motivation to live independently in the community or obtain adequate long-term supportive housing arrangements within the community. Residents are expected to pay no less than 30% of their income for rent and utilities, plus a service fee. (24-hr. on-site staffing) 3rd Fl. Downtown YMCA. 3. 12th St. Safe Haven – (25 single room occupancy units): Same as Safe Haven @ Y and predominantly chronic homeless. 12 E. 12th St. Contact: Connally Perry (Safe Haven Admin.), or, Dale Bacher (Team Leader): 584-2057

4. Walker Hall Transitional Living Center (12 bed congregate living): 100% for homeless adult individuals with mental illness who agree to enter into a transitional living program that has a “high demand/high expectation” philosophy of care, with the expectation that the individual will be ready to successfully move into independent housing within two years. Residents are expected to pay no less than 30% of their income for rent and utilities plus a service fee. (24-hr. on-site staffing) 1729 S. 5. Altamont Apartments (16 efficiency units): 100% of the units are dedicated to adult individuals with mental illnesses who need or prefer long-term supportive or independent living housing in the community that has 24-hr. security on-site. Current rents are $250 – 380/mo. including utilities. 6. Baltimore Apartments (16 studio units): 50% of units dedicated to formerly homeless adult individuals who have graduated from a transitional living program or are otherwise capable of living independently in the community. E ach unit is 450 sq. ft. with kitchen and bath. Current rent is $380/mo. including utilities. 7. Treepoint Apartments (19 one-bedroom units; 1 two-bedroom unit): 50% of units dedicated to adult individuals with mental illness who have demonstrated the ability to live independently in the community. This program is not limited to homeless or formerly homeless individuals. Residents are expected to pay no less than 30% of their income toward rent and utilities with Sect. 8 assistance. Currently rent is $454/mo. including utilities. Some subsidy may be available and Sect. 8 is accepted. 1115 S. College Ave. Contact: Gary Goldberg (Housing Specialist/Leasing Manager) 382-2475

8. Terrace View Apartments (24 one-bedroom and 18 efficiency units): 40 of the units have Section 8 Rental Assistance through a HAP contract with HUD wherein the occupancy is limited to lower income individuals who must pay 30% of their income toward rent and HUD pays the balance. Up to half of these units are available for long-term supportive or independent living units for persons with serious mental illnesses (homeless or non-homeless). 9. Bradstone Apartments (1 efficiency, 8 one bedroom and 2 two-bedroom units): Purchased in late 2005, the Bradstone Apartments are similar in operation to the Treepoint and Baltimore Apartments, with at least 8 of the units being dedicated to homeless or chronically homeless persons with serious mental illnesses. Some subsidy may be available through MAP-LTS for homeless persons, and Sect. 8 will be accepted. Currently rent is from $330 - $550/mo. including utilities. 1218 – 1220 S. Quaker. 10. Belle Arms Apartments (2 efficiencies, 10 one bedrooms and 8 two bedroom units): Purchased in June 2006, the Belle Arms will also be similar in operation to the Treepoint, Baltimore and Bradford Apartments, with at least 12 of the units being dedicated for homeless, chronically homeless or homeless prevention. Currently rent is from $325 - $475/mo. Some subsidy may be available through MAP-LTS for homeless persons with serious mental illnesses, and Sect. 8 will be accepted. 3732 S. Indianapolis. 11. Ranch Acres Manor (24 units, 20 one bedroom, 4 two bedrooms) Ranch Acres Manor is one of two properties purchased in 2007. up to 12 of the units will be dedicated to chronically homeless individuals. Currently rent is from $325 - $475/mo. Some subsidy may be available through MAP-LTS for homeless persons with serious mental illnesses, and Sect. 8 will be accepted. 3727 S. Indianapolis. 12. Southwind Apartments (8 units, one bedroom apartments) Southwind Apartments is the other new Association Property. Currently rent is $450/month utilities included. Some subsidy may be available through MAP-LTS for homeless persons with serious mental illnesses, and Sect. 8 will be accepted. 3719 S. Indianapolis. 13. Charan Apartments (20 units, 12 one bedroom, 8 two bedrooms) Purchased in 2008, Charan Apartment is one of the newest properties owned by the Association. The Charan is the first property acquired under the Building Tulsa Building Lives initiative to end chronic homelessness in Tulsa with up to 10 of the units being dedicated to the homeless. Currently rent is from $375 - $575/month. Some subsidy may be available through MAP-LTS for homeless persons with serious mental illnesses, and Sect. 8 will be accepted. 3230 S. Winston. 14. Indianapolis Apartments (16 1-bedroom units) Purchased in mid 2008, the Indianapolis is the second property acquired under the Building Tulsa Building Lives initiative to end chronic homelessness in Tulsa with up to 8 of the units being dedicated to the homeless. Current rent is from $400 - $460/month. Some subsidy may be available through MAP-LTS for homeless persons with serious mental illness, and Sect. 8 will be accepted. 3701 S. Indianapolis. 15. Ritz Apartments (10 studio, 13 Efficiencies and 3 1 bedroom units) Purchased in September of 2008, the Ritz Apartments is the newest property owned by the Association. This is the third property acquired under the Building Tulsa Building Lives initiative to end chronic homelessness. Up to 10 of the units are dedicated to help replace the Downtown YMCA SRO program and other units will be dedicated to formerly chronic homeless persons. Current rent is from $350 - $500/month. Some subsidy may be available through MAP-LTS for homeless persons with serious mental illness, and Sect. 8 will be accepted. 2601 E. 15th Street. 16. Metropolitan Apartment Program/Long-term Supportive Lite (LTS-Lite) and LTS Apartments (Supportive services-scattered units within sites 5 thru 9 above): 100% dedicated to homeless adult individuals with mental illness who have not demonstrated the ability to live independently in the community with normal outpatient support, but do not require 24-hr. on-site support. 17. YMCA Case Management and SRO Program (Single Room Occupancy and Case management services): This program which began in 2005 is designed to help single adults living at Tulsa’s Downtown YMCA access the services they need to be able to move into a more permanent housing situation. These needs include providing referrals and access to services for mental health care, medical care, dental care, employment, vocational training, legal services, benefits application, food, clothing and transportation. Through this program the Association controls access to 6 additional SRO units at the Y. Rent at the YMCA is $282.00 - $304.00 per month. Contact: Patty York (YMCA Case Manager) 582-4515.

18. Partnership for Open Doors (Home ownership): A Model program in partnership with Habitat for Humanity Tulsa, the Partnership for Open Doors helps families and individuals with mental illnesses move into home ownership. Families are screened and accepted into the program through the Association who helps Habitat develop an individualized plan for each family to complete the required sweat equity hours necessary. The financial requirements to qualify for a mortgage are designed on a disability income. As of 2006 we have moved three families into their own homes with more applications being accepted for 2007.
