Mission Medical is a faith based non profit organization that provides free healthcare to uninsured, under-

insured, and low income members of the Pikes Peak Region. On site volunteer doctors, optometrists,

dentists, mental health providers, diabetes specialists and chaplains offer their services to those in need.

The services we offer include primary care, dental, optometry, behavioral health, diabetes care, spiritual

care, and the ability to offer over the counter and prescription based pharmaceuticals. We can only see

patients that have been seen by one of our Volunteer providers. We provide health care for those who do

not have insurance or are under insured. Mission Medical (MMC) has a fully functioning Pharmaceutical

Dispensary to provide our patients free prescriptions. As a provider prescribes for a patient it is a one stop

service.

Mission Medical is undergoing a long overdue upgrade to the pharmacy/dispensary. This will be a complete new Pharmacy Management System and accompanying computer hardware systems. This upgrade will allow the pharmacy volunteers to fill prescriptions faster and allow MMC patients to check for drug interactions and get refills on-line. The Diabetes Clinic at Mission Medical Center is staffed by a volunteer team is led by a renowned endocrinologist. The clinic includes a podiatrist, nurses, dietitians, and diabetes educators. In addition to focusing focusing on treating patients with diabetes, the team provides regular care and education and tips on how to manage this disease. This affords patients a means to potentially avoid complications that can be deadly.

Summary:

This position allows interns to work in three unique areas; Pharmacy Admissions and Transitions of Care Services, Atrium Outpatient Pharmacy and the Medication Access and Renewal Center (MARC). Based on individual performance, the successful intern will have the opportunity to work closely with pharmacists and nurses to provide clinical services in these areas. There will be opportunities to work on projects to complement your academic learning and increase your clinical practice experience.

Job Duties:

Pharmacy Admissions and Transitions of Care Services ? Perform medication reconciliation on newly admitted patients in order to reduce inpatient medication errors ? Address discharge medication barriers ? Assist with discharge medication reconciliation ? Conduct discharge medication counseling or counseling about the patient's disease state ? Provide a list of the patient's discharge medications to their community pharmacy ? Follow up with the patient telephonically 2 days after discharge

The pharmacy department is currently seeking a P1 or P2 pharmacy student for an intern position. Per State and Federal Law and Craig Hospital Policy and Procedure, under the supervision of a licensed pharmacist, the Pharmacy Intern may complete all duties of a licensed pharmacist, except the final check and dispensing of medications.
