Enrollment Procedures

• The WAS Health Plan includes major medical, prescription drug card, and basic life insurance. In addition, the Plan offers optional dental, vision care and voluntary life insurance.

• All eligible employees must be covered by the basic life insurance and this is a board of education paid benefit.

• A new employee and eligible dependents should be enrolled within the first thirty-one days of employment. Coverage begins on the first day the employee reports to work.

• A new employee must complete an enrollment and voluntary life insurance application. Once the application has been completed, please enter the data on Consociate’s web portal. The original application should be mailed to the Eligibility Department at Consociate, Inc. The district/agency should retain a photocopy in their files of all enrollment forms and receipts.

✓ An employee can elect to be covered for single or family (employee+1, employee+2, employee+3/more) health coverage. Family coverage includes an eligible spouse and/or children.

✓ An employee can elect single, employee+1, employee+2 or employe+3/more medical/Rx card, dental or vision care. To elect dental or vision care, the employee must elect the medical/Rx card. The employee must also elect dental and/or vision to be able to elect any level of dental and/or vision for their dependents. An employee can elect different levels of dental or vision care regardless of the level medical/Rx care they elect, (i.e., an employee can elect single medical/Rx and employee+3/more for his dental coverage.

✓ An employee may waive all medical/Rx, dental and vision care. To waive all coverage, please complete the waiver of coverage portion of the application.

• Each new employee should be provided a plan booklet, a basic life insurance certificate, and a voluntary life insurance certificate (only if the employee elected voluntary life insurance).

• A newly hired employee should also be given copies of:

1) PPO explanation

2) Initial Women’s Health and Cancer Rights Act notice

3) Initial open enrollment notice

4) Initial Notice of Privacy Practices/HIPAA

5) Initial Notice of Special Enrollment Rights

6) Summary of Benefits and Coverage (SBC)

(If you offer the Red Plan, please distribute the $1,000, $2,000 and $3,000 HSA SBC)

7) HRA plan booklet, amendments and SBC (if applicable)

8) IL insurance exchange notice

9) Medicare Part D – Creditable Coverage

10) ServeYou – prescription drug information

11) Consociate claims analysts

13) MeMD information

• Ensure the employee signs a receipt for these informational items.

• If dependent medical coverage is elected, and a spouse is covered, a spousal COBRA notification must be mailed to the spouse by the district/agency.
