|JOB SUMMARY |

| |

|The post holder will work as a leading member of the Emergency floor team in Carlisle. In the Senior Primary Care Nurse role, |

|the post holder will be responsible for recognising negotiating and implementing change processes to enhance the delivery of |

|care. |

| |

|Patient centred care will be delivered by assessing patients with undifferentiated and undiagnosed problems, initiate |

|treatment, investigations, advice and management of problems using professional autonomous decision making processes, liaising|

|when appropriate with other healthcare professionals and service providers to ensure safe and effective patient care. |

| |

|The post holder will contribute to development and delivery of a high quality innovative service to the community through the |

|implementation of evidence based clinical practice, supported by continual professional development, and provide support and |

|mentorship to new staff, students and other healthcare professionals. |

|The function of the department is to provide prevention of admission (where safe to do so), facilitate early discharge and |

|treatment of patients closer to home. |

|KEY WORKING RELATIONSHIPS |

|Clinical care is provided by a team comprising of Primary care nurses, Nurse Practitioners and General Practitioners (GP’s), |

|supported by administrative staff and drivers who work together to deliver Primary care in the out of hours periods. |

| |

|The primary care nurse/practitioner always works independently and where appropriate, in collaboration with other healthcare |

|professionals. |

| |

|The Primary Care Practitioner regularly access other organisations such as A&E, CHOC, short term nursing, rehabilitation and |

|social work, Community services and wards within the local Hospitals. Mental Health services, District nursing services, Rapid|

|Response nursing services, Child protection services, Pharmacy services, Stores and equipment departments, NHS Direct and |

|other local helpline agencies. |

|DUTIES AND RESPONSIBILITIES OF THE POST |

|Communication; |

|Communicates effectively as a member of a multi-disciplinary team. |

|Refers when necessary to general practitioners, Paramedics, Accident and Emergency, Secondary care, District nurses, Night and|

|Day rapid response nurses, Emergency social workers, Emergency mental health team, Emergency dental services, Rehabilitation |

|team and CHOC. |

| |

|With Primary care nurses and community teams giving, support, advice and leadership. |

| |

|With GP’s on a daily basis for prescriptions, medication advice and collaborative working. |

| |

|With reception staff regarding patient information. |

| |

|With other primary care trust employees for all manners of business, clinical supervision, continuing professional |

|development, clinical governance issues, meetings for development of the service. |

| |

|With acute hospital staff on a daily basis regarding patient care referrals and best practice for maximum benefit. |

|Clinical skills; |

|Works independently as a Primary Care Nurse/Practitioner within the scope of professional practice. |

|Functions at a high level to carry out clinical decision making such as diagnosing and discharging patients without input from|

|a medical practitioner remaining within the scope of professional practice |

|Practices as an autonomous practitioner where possible with non medical prescribing authority |

|Functions at a high level in relation to communication skills through patient consultation and nursing assessment of physical,|

|psychological and social needs. |

|Takes an accurate history, building a rapport towards a therapeutic relationship. |

|Assess plan and implements patient centred plan of care for patients within the out of hours treatment centres and refer when |

|necessary to other healthcare professionals for further specialised care |

|Clinical skills; |

| |

|Adapts to change within the working situation managing unscheduled emergency situations. |

|Basic life support, severe breathing problems, serious cardiovascular problems, collapsed patients and seriously ill patients.|

| |

|Demonstrates clinical competence in the clinical care management of patients of all ages from infants to the elderly, |

|pregnancy related problems and management of lifelong illness such as diabetes, asthma and chronic obstructive pulmonary |

|disease during the time of acute illness |

| |

|Performs safe manual handling techniques. |

| |

|Delivers a high standard of care without discrimination, to all ages, sexes and ethnic origins of the population through |

|continuum of life. |

| |

|Performs investigations required to reach a differential diagnosis using core clinical skill and competence. Initiate |

|treatment including diagnostic investigations. |

|Recognition of abnormal vital signs observing paediatric and adult respiratory rate and effort, heart rate rhythm and volume, |

|temperature and blood pressure as appropriate. |

|Perform investigative procedures including; Performing ECG’s, Urinalysis, |

|Blood Glucose monitoring, Venopuncture, measuring and interpreting Peak Flow |

|Demonstrates competence in examination techniques |

|Examination of the Ear Nose and Throat system |

|Basic Neurological examination of Headaches and Head injury |

|Basic Musculo-skeletal examination in the assessment of Injury |

|Basic eye examination in the assessment of injury, foreign body and infection. |

|Proficient in treating and managing common minor illness |

|Deliver safe, effective evidence based care and advice to ease patients problems |

|medical independent prescriber. |

|Independently discharge patients from your care or refer to the appropriate health Professional or healthcare provider for |

|further care intervention. |

|education to aid adjustment of lifestyle, including Contraception advice |

|Request diagnostics (Including x-rays) following further training. |
