Fort Polk Spouses’ Club 2021 Merit Scholarship ApplicationDear Applicant,This is an application for scholarships awarded by the Fort Polk Spouses’ Club for 2021.Eligibility CriteriaApplicant must be an immediate family member or dependent of an active duty, National Guard, or Reserve service member; retiree; or Gold Star family member. The applicant or his/her military sponsor must reside within a 100-mile radius of Fort Polk.Applicant must submit an authorized DEERS enrollment form (DD Form 1172-2) or proof of eligibility.The applicant must be accepted, or expect to be accepted, by an accredited institution of higher education, be it undergraduate, graduate or vocational. The applicant must enroll as a full-time student as defined by the university/college during the 2021-2022 academic year.Family members of either the scholarship review committee or the scholarship judging committee are ineligible.**Please note that the Fort Polk Spouses’ Club facilitates the First Command Bank Scholarship in conjunction with its own scholarship. If an applicant is awarded the First Command Bank Scholarship, he/she will not receive a Fort Polk Spouses’ Club Scholarship. Additionally, if a scholarship winner accepts an appointment to a military academy, any award must be forfeited and returned to the Fort Polk Spouses’ Club.RequirementsThe complete application packet.The applicant’s authorized DEERS enrollment form (DD Form 1172-2). This form may be obtained from the DEERS office. It must also be signed, dated and authenticated by an employee of the DEERS office. OR The applicant’s sponsor may access their DEERS profile through DS Logon - Login (osd.mil). They should then print the form verifying the applicant’s enrollment in DEERS. The sponsor must then sign and date the form.A copy of the most recent high school, college or vocational school transcript.One letter of recommendation from a source outside the applicant’s family. Acceptable sources for letters include, but are not limited to: school faculty member, employment supervisor, member of a volunteer organization, or a member of the clergy.A 8.5 x 11 inch paper, single spaced, one inch margins, with a minimum 500 word typed response to the question below. The essay must be written entirely by the applicant. It will be judged on content, grammar and punctuation.COVID-19 has changed the way we gather together, study, and learn. Many college students were unable to physically attend classes, attend study groups, or otherwise participate in sports and social activities. If COVID-19 restrictions are not relaxed in 2021, how will you maintain your academic discipline, quest for learning, and work to maintain a balance of physical, mental, and emotional health while in college? Sign and date the application, thereby acknowledging all statements to be true and factual and that you are fully aware of all the rules and requirements of said application.Application InstructionsApplication must be filled out using the electronic format downloaded from fortpolkspousesclub.. Transcripts, authorized DEERS enrollment form (DD Form 1172-2), completed essay, and references must be attached.Packets may be submitted in hard copy using a 10x13 or larger envelope. Applications will also be accepted electronically via email. If submitting by email, place your name in the subject line and attach a PDF file containing the COMPLETE application packet. Scholarship recipients will be recognized at a ceremony, TBD.Judging of applications is based on academic achievement (40 pts); essay (25 pts); community service, extracurricular activities, and personal achievement (20 pts); and recommendations (15 pts).Checks will be paid directly to the school and will be applied to the fall semester 2021. Each scholarship winner must notify the FPSC Scholarship Chairperson no later than May 1, 2021 with the current mailing address of the school he/she will attend.
