Call Outs and Requesting Off Policy When Working in School Districts

As an employee of Brett DiNovi and Associates (hereinafter referred to as BDA) you will be treated as a professional and expected to produce that quality of work. Your timely attendance to work is crucial to providing excellent services to our clients. In order for us to be able to provide excellent services necessary to various school districts & day programs, you need to do your part in maintaining a good attendance record.

Punctuality requirements. All BDA clinicians working within school districts & day programs are required to be at the designated school 15 mins prior to the student's arrival time in order to ensure the ability to meet your learner in time for a successful transition to the classroom. If you are to support the transition to the school, per the learner's IEP, then you are to be at the learner's home at the time designated to meet this goal.

Lateness. Should you be running late, you are to notify the assigned building administrator (this could be the teacher) and the Behavioral Consultant on the case no later than your required arrival time in addition to the call-out line. An average of 4 occurrences of lateness in a school year will be considered excessive. Excessive lateness can result in disciplinary action up to and including termination. In addition, an average of 4 or more occurrences of leaving more than 15 minutes prior to the typical end of the school day is considered excessive.

Call Out Procedure. In the event that you are unable to report to your assigned school you are to call Joe Carter no later than 3 hours prior to your required arrival time. This needs to be done in order to ensure that we will have adequate time to find coverage and give the substitute enough time to get to the appropriate school. Joe can be reached at 302562-7472. You are also required to follow the call out procedure as outlined in the BDA Staff Handbook.

Call Joe Carter 302-562-7472 notifying him of your district, name of learner, student drop off time, and any pertinent information the substitute will need to know in order to cover the shift.

Also call the call out line 856-628-7732 (used for both staff & client cancellations). Leave a voicemail stating: your name, date and time of shift or appointment canceled, the name of the district and client being canceled, confirmation that you notified Joe Carter. Excessive absenteeism is defined as 8 or more cancelations within one year period or 2 cancelations within a 30 day period.

Requesting Time Off. BDA is aware that there will be times when you will need to take a day off, planned vacations, weddings, etc. Should you desire to take time off during the school year, submit your request to Joe Carter 15 business days prior to the desired day off. Once the request is submitted we will evaluate if the request is able to be fulfilled and let you know within 3 working days if your request is able to be honored.

Email all requests to Joe Carter at Jcarter@ Requests submitted later than 15 business days prior to the desired day off may not be

approved. Note that these are requests for time off. Should there be the inability to fill your shift or

circumstance that are deemed by the leadership team as reason for denial, then your day may be denied. The company will respond to your request within 3 business days

o Your request may initially be denied but later approved due to substitute availability

We understand emergency circumstances out of your control may fall under the exception to the above listed protocol. Should circumstances like this arise, you are to NOTIFY JOE CARTER AND THE CALLOUT LINE as soon as you know you will not be making your shift.

We serve clients with very intensive needs, noncompliance with the above listed protocol may result in disciplinary action up to and including termination of employment. Also, cancellations and absenteeism that do not meet, "excessive absenteeism" may still result in the client's request to change consultants.

By signing below I am acknowledging my understanding of the above listed protocol.
