2016-17 Voluntary Student Accident Insurance InformationThe Chico Unified School District (CUSD) does not carry medical or dental insurance for students injured on school premises, while under school jurisdiction, or while participating in school district activities. For these reasons, the school district has arranged a student accident insurance program for your review and consideration. This insurance program is optional and complies with California Education Code, which permits distribution of necessary information from the vendor, Student Insurance (SI), providing the coverage. The following types of student accident insurance plans are available:School Time AccidentCost: Low Option - $14.00 Mid Option - $28.00 High Option - $43.0024 Hour AccidentCost: Low Option - $82.00 Mid Option - $105.00 High Option - $210.00Football AccidentCost: Low Option - $85.00 Mid Option - $115.00 High Option - $215.0024 Hour Dental Accident - $8.00 For additional information including coverage summaries and registration details, please visit the CUSD Parent/Community Forms website (reference: Student Accident Insurance Coverage 2016-17) at: Participation in these insurance plans is voluntary. Approved claims will be paid on a primary basis for all school-time and 24-hour injuries, except tackle football (paid on an excess basis). Although you may already have medical insurance, purchase of this additional coverage could assist with deductibles and balances after primary carrier has paid.The application process will now be done online, and will allow parents to safely and securely transmit applications through one of the following methods (registration details may be found at CUSD Parent/Community website (): Direct online submission: , online application may be completed and submitted securely on website. Insurance will be effective immediately as well as policy number and insurance card. System is equipped to accept credit, debit, and e-checks for parent’s convenience. Web “download”: , may complete and print application from website. Certified funds are available upon receipt of hardcopy application and cashier’s check or money order for policy to be effective. Applications will become effective once received in the SI office and policy number available.
