Job DescriptionTitle of Post:Habilitation Specialist / Rehabilitation Worker for Visually Impaired PeopleGrade / Salary?25 113 - ?32 842 pro-rataHoursFlexible (up to 3 days a week)Location: 14 New Row, Perth or home basedReports to:Client Services ManagerJob PurposeTo provide assessment and habilitation services to visually impaired children and young people (with an emphasis on mobility), in order to maximise their independence, opening their way in the future to further study, employment and independent lives. Key TasksDelivery of a holistic Habilitation assessment and support service with an emphasis on mobility. The assessment of needs would then direct the input. To undertake an holistic needs assessment using agreed methodologies, recording information as required and identifying / agreeing key outcomes, using a person-centred approach.To develop support plans setting out how key outcomes will be achieved. These are likely to relate to optimising and improving one or more of the following areas of habilitation:independent livingindependent travel and mobilityemotional well being including self confidence and self esteemsocial inclusionuse of specialist habilitation tools as aids to mobility and independenceability to assess risks and anticipate likely areas of personal difficulty in mobility and independent living.To provide advice, information and support to parents, carers and other relevant professionals, embedding and reinforcing the impact and effectiveness of any training programmesWrite reports and keep records of children and young people’s progress during training, in line with VisionPK’s systems.6. To monitor outcomes and impact, and identify any further training needs7. Attend termly CVISTA Meetings8. Provide regular monitoring and evaluation data in the form of quarterly written reports and an Annual Report, detailing activities undertaken, impact information, and service users’ views. Reports will include the following:-Meetings attendedNumber of information/advice phone calls loggedPublications distributedIdentification of areas of needOutcomes achievedProgress regarding uptake and input of training opportunitiesStatistical information indicating progress achieved towards targetsCase scenarios and feedback receivedGeneral ResponsibilitiesTo work within, and adhere to, the policies and procedures of VisionPK, and to contribute to reviewing these when appropriate.To undertake such duties appropriate to the level of the post as may be reasonably required to meet changing needs of the organisation.To adopt a flexible approach to working hours to meet the needs of service users and carers.To support a culture of continuous improvement and equality in the provision of services.To manage and be accountable for, with supervision and managerial support, your own practice within the organisation, including maintaining professional development.
