Personal Care Attendant/CAI/CLS

Job Description

Part Time 35 hours/week

General Responsibilities

• Observes center’s policies and procedures

• Maintains accurate time sheets and uses leave appropriately

• Maintains client and center confidentiality

• Willingness to accept responsibility and take initiative to get the job done

• Maintains clean, safe work environment

• Participates in staff meetings, in-services, and training sessions

• Completes work on time

• Can be relied upon to work when and where assigned

• Maintain open and working dialogue with co-workers and supervisors

• Writes GER/incident/accident reports

• Assist with client care (feeding, hygiene, and toileting ) as needed

• Assist with transportation as necessary

Specific Job Duties-CLS (This position will be supervised by an RN employed by MSC)

• Assist consumer with bathing, toileting, grooming, shaving, dental care, dressing, and eating and may include but is not limited to proper nutrition, home management, housekeeping tasks, ambulation and transfer

• Transport consumer to places outside their residence for purposes such as appointments, entertainment, exercise, recreation, shopping or social activities

• Assist consumers in various household or housekeeping tasks necessary for the upkeep and cleanliness of the home including but not limited to such activities as doing laundry, changing linens, trash disposal and cleaning

• Assist consumers with grocery shopping and meal preparation

• Assist consumers with making appointments

• Supervise field trips or community outings

• Duties may include proper nutrition, home management, house keeping tasks, ambulation and transfer, and medically related activities, including the taking of vital signs only in conjunction with the above tasks.

• Provide training of goals and objectives of the Service Plan and ISP

• Maintain tracking records of ISP and Service Plan

• Report any needs, change in consumer condition or observed problems that affect the client on the Daily Documentation and verbally to the supervisor

• Other duties as assigned by supervisor.

Community Access Individual Services

• Schedule Individual outings with consumers on a monthly basis

• Look for new volunteer opportunities in the community

• Look for new activities for consumers to match their interests and needs

• Supervise individuals while on outings or volunteer jobs

• Document CAI services

While at the center and not on CAI services it is expected that this position help out with CAG and Pre-voc activities at the center as needed and to document these services.

Facility

• Participate in development of individual ISP

• Train Client on desired outcomes identified in ISP

• Assist Supervisors to develop instructional activities for training of desired outcomes

• Maintain tracking records of training in a timely manner

• Maintain good communications with family/care providers

Job Requirements

• High School Diploma or GED

• CNA

• Pre-employment and random Drug Testing

• Criminal Records Investigation

• Driving records check

• Annual training as determined by the Annual Training Needs Assessment

• Must be willing to use own vehicle in performance of job duties when necessary
