Patient Access Lists

Introduction

The Patient Access List (PAL) is the online work and rounds list used in eKiDs PowerChart. A clinician uses a point-of-care device or a workstation to access the PAL.

The PAL helps clinicians work effectively by providing key patient and workflow information in an easy-to-access format. From the PAL, clinicians can reschedule tasks, chart results, and access a patient’s chart. This functionality provides a single source for performing a wide range of functions.

Accessing the Patient Access List

When you log into eKiDs PowerChart, it will open to the Patient Access List.

Establishing a Patient-Provider Relationship

Before gaining access to any patient’s record, you must first illustrate that you have a need for patient information by establishing a Patient-Provider Relationship.

When you access the PAL at the beginning of each eKiDs PowerChart session, the Patients without Relationships window will display, allowing you to establish a Patient-Provider relationship with patients on your patient list with whom you do not have a relationship.

Establishing a Relationship

When prompted to establish a relationship with a patient, complete the following steps:

1. In the Establish Relationship window, select a relationship type from the drop-down list.

2. In the Patients without Relationships section, the names of patients on the Patient List that you have not established a relationship with are displayed. A check box is displayed to the left of each name. The system defaults to selecting all the patient names on the list. Deselect patients whom you do not want to establish a relationship with by un-checking the box.

3. Click OK.

Considerations

• Deselecting patient names in the Patients without Relationships section allows you to apply different relationship types to different patients.

• If you de-select patient names and close the Patients without Relationships window, no relationship will be established with those patients.

• A patient with whom no relationship was established can be displayed in your PAL, but their tasks, demographics, and results will not be listed in your PAL until after you have established a relationship with them.

Understanding Patient Access List Components

There are several sections available in the PAL allowing the clinician to chart tasks, display results, retrieve additional information and access the patient’s chart.

PAL Sections

The Patient Access List is divided into sections, making the ability to locate pertinent patient information a simple process. The sections are not labeled. Instead, they are identified by the information that they contain.

Individual columns within sections are labeled and display icons or text that link to information relevant to the patient. Each column within a section displays a different type of information, such as room location, blood pressure, or results.

|Name Section |Contains the list of patients whose information can be accessed. This |

| |section is always displayed on the left-most section and is set apart from|

| |other sections by its light green background. |

|Demographics Section |Contains selected patient demographic information such as Location, Age, |

| |VIP Indicator, and Allergies. Additional columns can be added to this |

| |section, based on your personal preferences (see the Column Customization |

| |within the Patient Access List section). |

|Task List Section |Contains selected patient task information such as Overdue, PRN / |

| |Continuous, or Current Tasks. |

| |This section displays a set of task group icons for specific tasks within |

| |your system. |

|Results Section |Contains a New Orders Indicator, signaling that a new order has been added|

| |to the system for the selected patient. Also contains a Results Indicator |

| |that allows you to access test results information for your patient. |

| |Additional columns can be added to this section, based on your personal |

| |preferences (see the Column Customization within the Patient Access List |

| |section). |

|Information Bar |Displays your selected Patient List to the left and shift information to |

| |the right. Both the shift and the Patient List that are displayed on the |

| |Information Bar can be changed. |

Information Bar

The Information Bar is a blue bar that sits above the four sections in the Patient Access List. The Information Bar displays the selected Patient List on the left and the selected shift on the right. The shift and the Patient Access List that are displayed on the Information Bar can be changed based on personal preference.

Name Section

The Name Section in PAL appears in the far-left position of the PAL. The listing of the patients that displays in the PAL is selected by the end-user from any patient lists that he or she has created. The list of names that appear in the Names Section of the PAL are charts that can be opened from the PAL.

Demographics Section

The Demographics Section of the PAL contains selected Patient Demographic Information such as Location, Visit Reasons, Attending Physician, Allergies and IV information if applicable.

Considerations

• Within the Demographics Section, there is an Allergy column that displays at-a-glance allergy information for the patients in the PAL. Simply follow the Patient’s name across to the Demographics Section until you get to the Allergies column. Depending on the icon that is listed in the Allergies column, you can determine if the patient has allergies recorded, no allergies recorded, or no known allergies.

• Double clicking on an Allergy icon will open the Allergies window where a clinician can view Reaction, Severity, Category and Substance Information related to a patient’s allergies.

Task List Section

The Task List Section of the PAL contains at-a-glance task information for patients listed in the PAL.

A clinician can quickly view which tasks have Overdue, PRN / Continuous, or Current statuses. In addition, links to documentation and task completion can be obtained in the Task List section by clicking on the icons.

A heart icon indicates nursing tasks and a lungs icon indicates respiratory tasks. Nursing and Respiratory will be able to view all tasks but can only chart on the appropriate one for their discipline.

Results Section

The Results section in the PAL contains two default columns: New Orders and Results.

The New Orders column will display an icon when a new order has been added to the system for the selected patient.

The Results column will display an icon when new Result Information has been posted for the selected patient. Critical results will display in red.

Additional columns can be added to this section, based on your personal preferences (see the Column Customization within the Patient Access List section).

To display the last charted result details, complete the following steps:

1. Double-click the desired icon in the Results Section to display the Results Details window, which lists detailed information for the patient that you have selected.

Depending on the status of the result, the user may see various result statuses listed in the Result Status column of the Results window:

2. From within the Results window, right-click selected Result and select View Details to view detailed result information.

3. The Result Details window will appear. Click on the appropriate tab to obtain additional information regarding a charted result.

Patient Access List Toolbar

The Patient Access List toolbar allows users to manage the patients that are being displayed within the Patient Access List. The usage of the Patient Access List toolbar in customizing the displayed Patient List within the Patient Access List will be discussed in this manual later.

Each icon that appears in the Patient Access List toolbar and its functionality are listed below.

Tasks Assignment Procedure

In order for a clinician to successfully manage assigned tasks, it is important to understand the task assignment procedure.

Workflow Process

Tasks get on the Task List by any of the following methods:

1. An order is placed. (Not all orders create tasks)

2. Completed order details in PowerChart have triggered the system to create a task.

3. Specific orders from PRN interventions cause a PRN Response Task to be triggered "X" number of minutes after the intervention is administered. [pic]

Timeframe Selection

The Timeframe Selection window allows you to select the appropriate shift to display your assigned tasks.

When you first open the PAL after log-in, the Timeframe Selection Window is automatically displayed.

To display the Timeframe Selection window in the middle of a PAL session (after a Timeframe has been previously selected):

1. Right-click on the Information Bar within the PAL.

2. Select Change Timeframe.

3. The Timeframe Selection window will display.

To define a time frame in the Timeframe Selection window, complete the following steps:

1. Select the appropriate shift by clicking on the correct shift.

OR

2. If your assigned shift is not listed in the Select a Shift section, you can manually type in the date and time frame using the Select a Time Range option within the Timeframe Selection window.

a. Click on the Radio Option beside Select a Time Range.

b. Manually type in the Time Range in the To and From fields of the window.

c. To display the calendar, click on the drop-down arrow next to the date field.

d. When you are finished entering the timeframe in the Timeframe Selection window, click OK.

Tips and Tricks

• The Timeframe Selection window is time sensitive, and will only display multiple shifts when the current time can be applied to multiple shifts.

• You can quickly enter the current date and time in the From and To fields by entering a “T” in the date field (for today’s date) and an “N” in the time field (for the current time).

Setting Timeframe Defaults

To save time when using the PAL, Timeframe Defaults can be set. When the defaults are set, the user is not prompted to manually select a timeframe each time that they access the PAL.

To set Timeframe Defaults, complete the following steps:

1. From within the PAL, click on the Options menu.

2. Choose Set Defaults.

3. The Set Defaults window will display.

4. Choose either a Default Shift or Scope by clicking in the Radio button beside the appropriate option.

5. Click Apply to save and apply your changes.

6. Click OK to close the window.

Tips and Tricks

• The Select a Scope option will show patient data ranging from 1-72 hours before and after the current time. So, if the Select a Scope option was selected and a user entered 48 hours, the system would show patient data that spans from 48 hours in the past to 48 hours in the future.

• There is a limit of 1,000 tasks that can be retrieved. For this reason, you should not request the system to retrieve a large number of tasks (i.e. more than three months).

Charting Tasks within the Patient Access List

Once a task that is listed in the PAL has been completed, it is necessary to document its completion. If a task needs to be rescheduled or if the task will not be completed, this should also be documented.

| |Use this selection to perform simple charting for tasks that do not require|

| |Use this selection to chart tasks as done with a date and time to stamp on |

| |the results. You can also use this option to chart a task for someone else.|

| |Use this selection to chart a task that cannot be completed and will never |

| |be completed (example: the task is not appropriate for the patient). |

| |Charting a task as ‘Not Done’ will complete the task and remove it from the|

| |schedule. |

|Reschedule this Task | |

| |Use this selection to change the date / time that a task is scheduled to be|

| |completed to a later date / time. Task rescheduling parameters vary by |

| |department. (Refer to the Task Rescheduling Parameters for more information|

| |[pg 31]) |

|Un-Chart | |

| |Use this selection to change a task back to Pending and undo the previous |

| |charting. This may be necessary if charting was done for the wrong task or |

| |for the wrong patient. Any documentation that was associated with the |

| |previous charting will be changed to In Error.(Tasks must be uncharted from|

| |the Task List) |

|Create Admin Note | |

| |Use this selection to add an administrative note regarding a task. |

Charting Tasks as Done

To chart a task as done (performed / completed), complete the following steps:

1. Select the task for your patient from the Task Section of the Pal by double-clicking on the associated icon.

The Tasks window will open, displaying tasks for the selected patient.

2. Click the “Chart” button to chart the task as completed.

Once the task has been charted as complete, the task will no longer display in the PAL.

3. The Tasks window can be closed by clicking on the ‘X’ in the upper right-hand corner of the window. If you have not chosen to chart a task as completed, closing the window will not chart the task.

Tips and Tricks

• If there are several tasks for a patient, all will appear in the window. As a default, all tasks in the window will be automatically selected. If all tasks have been completed, they will be charted at the same time by clicking on the ‘Quick Chart’ or ‘Chart’ buttons.

• If there are some tasks in the window that you do not want to chart, remove the checkmark from those tasks, and only chart the tasks that have been completed.

• Several tasks can be charted at the same time as long as they can be charted by the same method.

Charting Tasks as Done (Date / Time) (not available for all tasks)

To chart a task as done with a Date / Time stamp, complete the following steps:

1. Select the task for your patient from the Task Section of the PAL by double-clicking on the associated icon.

The Tasks window will open, displaying tasks for the selected patient.

2. Right click on the task within the Tasks window.

3. Choose Chart Done (Date / Time) from the context menu.

4. When the Task window opens, enter the date and time that the task was completed in the ‘Date / Time’ field.

5. If the task was performed by someone other than the person who is charting the task, enter who the task was performed by in the ‘Performed by:’ field.

6. When finished, click OK.

Task Information will show that the task was completed at the date that was entered and performed by the person that was entered.

Un-Charting Tasks

To correct a task that was charted in error, you must access the SPTL or MPTL from the patient’s chart. You cannot un-chart from the PAL. This will be covered in the Task List section.

Rescheduling Tasks

To reschedule a task to be performed at a later date or time, complete the following steps:

1. Select the task for your patient from the Task Section of the Pal by double-clicking on the associated icon.

The Tasks window will open, displaying tasks for the selected patient.

2. Right click on the task within the Tasks window.

3. Choose Reschedule This Task from the context menu.

4. When the Reschedule Task window opens, enter the new date and time that the task is to be completed in the Rescheduled Date and Time section of the window.

5. Specify a reason that the task is being rescheduled in the Rescheduling Reason drop-down box.

6. When finished, click OK to close the window.

Considerations

• If the new reschedule date and time falls beyond the 24-hour rescheduling parameters that have been established, the OK button will remain grayed out until the date and time falls within the defined parameters.

• Selecting Reschedule this Task from the Tasks window applies only to the task selected. All other tasks within the Tasks window will remain in their current status.

• Rescheduling a task has no effect on subsequent tasks.

• If several tasks need to be rescheduled, cancel the original order and re-enter the order with the correct task times.

Charting Tasks as Not Done

To chart a task that has not been (and will not be) completed, complete the following steps:

1. Select the task for your patient from the Task Section of the PAL by double-clicking on the associated icon.

The Tasks window will open, displaying tasks for the selected patient.

2. Right click on the task within the Tasks window.

3. Choose Chart Not Done from the context menu or from the Chart Not Done button

4. When the Chart Not Done window opens, specify a reason that the task will not be completed in the drop-down box. If the reason is not listed in the drop-down box, select Other from the drop-down box and then free-text the reason into the Comment section.

5. When finished entering a reason why the task will not be completed, click on the green checkmark to save your information and close the window.

Creating Admin Notes

An admin note is a provider-to-provider communication such as "Do not allow the patient to walk unassisted" that displays on the PAL. Admin Notes are not permanent, but associated with a specific task. Therefore, when the task that the Admin Note is associated with is charted, the Admin Note is no longer displayed. Only one Admin Note icon is displayed beside a task, but multiple entries may be displayed on the same note.

Since Admin Notes do not have a date/time/name stamp, it is important to type in your name and the date of the Admin Note in case another provider needs to contact you regarding the Admin Note that you entered.

To create an administrative note that is applicable to a specific task, complete the following steps:

1. Select the task for your patient from the Task Section of the PAL by double-clicking on the associated icon.

The Tasks window will open, displaying tasks for the selected patient.

2. Right click on the task within the Tasks window.

3. Choose Create Admin Note from the context menu.

4. When the Admin Note window opens, enter the Admin Note.

5. Click OK to close the Admin Note window.

The Admin Note will appear next to the Task in the Tasks window. Any user can double-click on the Admin Note icon to view the Admin Note.

Considerations

• Admin Notes are viewable to all users who access the task.

• Once a task is completed, any Admin Notes that are associated with the task no longer display in the Tasks window.

• An Admin Note can also be created from the Single Patient Task List by right-clicking on the task and choosing Create Admin Note.

Miscellaneous Actions within the Patient Access List

Other options within the Patient Access List that can provide the user with additional capabilities include printing, viewing order information, and viewing task information.

Printing the Patient Access List

To create a screen print of the Patient Access List:

Select Print from the Task Menu.

Considerations

• Remember, you can print the PAL if you just need to carry a list of tasks, but Task Lists are updated immediately when a new order is placed, and a printed list may become very quickly outdated.

Viewing Order Information for a Task

To view Order Information for a Task:

1. Double click on the task in the Task Section of the PAL.

2. When the Tasks window opens, right-click and select Order Information.

3. Select the appropriate tab to view order details, comments, additional information, history, and results.

Considerations

• If the additional information that you have selected for a task is not available [or has nothing listed], it will be grayed out.

Viewing Order Comments

To view Order Comments:

1. Double click on the task in the Task Section of the PAL.

2. When the Tasks window opens, right-click and select Order Comments.

3. The Comments tab of the Order Information screen will display.

Considerations

• If Order Comments were not entered for the order that generated the task, the Order Comments option in the right-click menu will be grayed out.

Viewing Prep Information

To view Prep Information for a task:

1. Double click on the task in the Task Section of the PAL.

2. When the Tasks window opens, right-click and select Reference Manual.

Considerations

• If the order that generated the task that you selected does not have a Prep [Reference Manual] associated with it, the Reference Manual option will be grayed out.

Viewing a Patient Snapshot

To view a Patient Snapshot:

1. Double click on the task in the Task Section of the PAL.

2. When the Tasks window opens, right-click and select Patient Snapshot.

Patient Allergy and Diagnoses information will display. Click Close to close the Patient Snapshot window.

Column Customization within Patient Access Lists

Adding Columns to the Demographics Section of the PAL

Additional columns can be added to the Demographics and Results Sections of the PAL, based on personal preference.

To add columns to the PAL, complete the following steps:

1. Right Click within the section and select Insert Column.

The Column Panel Wizard will display.

2. Select the Column Type that you would like to add.

3. Click Next.

4. Select the Column Type.

5. Click Next.

6. Select the Type of Detail Information that you would like to appear (Custom Reports) or None.

7. Click Next.

8. Enter the column width (ten is the default) and the Column Header (column name that you want display in the PAL).

9. Click Next.

The specifications window will appear based on the information that you entered.

10. Click Finish.

The Additional Column will appear in the PAL.

1. Double click on ALLSRVSECTS.

2. Double-click on the category of information to display (i.e. Laboratory, Clinical Info, etc.)

3. Double-click on the applicable sub-category.

4. Select the appropriate result information that you want displayed within the PAL.

5. Click Next.

6. Enter the column width (ten is the default) and the Column Header (Column Name).

The specifications window will appear based on the information that you entered.

Adding Columns to the Results Section of the PAL – Sticky Note Column

The Sticky Note Indicator is not currently used at CHKDHS.

Removing Columns in the PAL

Columns that you added to the Demographics and Results Sections of the PAL can be removed based on personal preference. The default columns within the PAL cannot be removed.

To remove columns from the Demographics or Results Section of the PAL, complete the following steps:

1. Right Click within the section and select Delete Column.

The selected column will no longer display within the PAL.

Sorting Columns in the PAL

Each column in the PAL section has a heading, which is either a descriptive name or an abbreviation for the displayed information. Column headings are defined by IS, and cannot be renamed.

To sort columns, complete the following steps:

1. Click a column heading to select the column. The names of patients who have information available in that column, along with their data in all PAL sections, will rise to the top in descending order (Z-A) for that column.

2. To display the information in ascending order (A-Z), click the column heading again.

Collapsing or Expanding a Section in the PAL

You can reduce or increase the width of any section to provide more viewing space for information in the other sections and otherwise modify the layout of the PAL to best serve your needs.

To collapse or expand a section, use one of the following methods:

• Right-click anywhere in the section you wish to collapse or expand. Select Minimize View to make the section narrower or select Maximize View to make it larger.

• Sections also can be resized by dragging one side or the other to the desired position using the mouse.

Troubleshooting / Patient Access List System Messages

The system messages listed below are displayed in the Patient Access List if an error occurs.

Troubleshooting Q & A

Q. Why would I want to pull a Patient List into my PAL or SPTL?

A: When a Patient List is selected to be used in the PAL or Task List, the Task List will display all tasks for patients that appear on that Patient List. This is different from the Departmental View, which shows all tasks based on your role (i.e. Physical Therapist, Diet Tech).

Q: Should I print my PAL or task list to make rounds?

A: Remember, you can do a screen print if you just need to carry a “current list” but Task Lists are updated immediately when a new order is placed and a printed list may be very quickly outdated. Therefore, printing a task list to complete tasks is not suggested.

Q: Some of the orders that I put in still show a status of Processing. Why aren’t these orders showing the correct status?

A: It is imperative to refresh your display by clicking on the As Of Button. Otherwise, the data that you are looking at may not be up-to-date.

Q: Why does the OK button remain grayed out when I attempt to reschedule a task?

A: The OK button remains unavailable unless the rescheduled date and time fall within the defined range. See the Rescheduling Parameters Chart (Page 31) for more information.

Q: Why aren’t tasks for my patient displaying on the Single Patient Task List?

A: If tasks are not being displayed on your task list for your patient, click the Refresh Button to confirm that you are viewing the most accurate information.

If tasks still are not displaying, check the date / time on your Information Bar. If the date on the Information Bar is not inclusive of the date of the task, then the task will not be displayed on the task list. (For more information on changing the date / timeframe for tasks, see page 24).

If you have confirmed that the date / time on the Information Bar is accurate, but the tasks are still not displaying, check the Task Display Filter to confirm that the Task List is set up to display all task status types. (For more information on changing the Task Display Filter, see page 39).

Q: I have a patient displayed in my PAL, but in the tasks, demographics, and results sections, ‘No Relationship’ is displayed. Why?

A: A patient with whom no relationship was established will be displayed in your PAL, but their tasks, demographics, and results will not be listed in your PAL until after you have established a relationship with them.

Q: An order that generated several tasks on my PAL needs to be rescheduled. What do I do?

A: Rescheduling a single task has no effect on subsequent tasks. If several tasks need to be rescheduled, cancel the original order and re-enter the order with the correct task times.
