INSTRUCTIONS: Submit all of the following:

⇨ This Cover Sheet: Complete the info at the top of this page, including the date you submit your completed packet to your counselor/teacher/administrator.

⇨ Parent/Guardian Brag Sheet: Have a parent/guardian complete the Parent/Guardian Brag Sheet thoroughly to help your counselor/teacher/administrator gain a better understanding of you

⇨ Brag Sheet: Complete the Student Brag Sheet with thorough responses (preferably typed). Keep in mind, the more detailed your answers, the better and more individualized your letter will be!

⇨ Personal Statement Essay: Attach a personal statement essay (if required by organization).

⇨ If the letter of recommendation must be mailed: For each university that DOES NOT use the Common App or Send-EDU, attach a pre-completed form, and a pre-addressed envelope with 2 stamps. (For large manila folders, use 4 stamps.)
