ATTACHMENT A TO PURCHASE ORDER #______STATEMENT OF WORK This Statement of Work (“SOW”) is issued pursuant to Purchasing Agreement #_______ dated _______________, 20__ between UC and Supplier (“Agreement”). Title and Description of the Scope of Goods and/or ServicesThis is a network equipment removal and reinstallation “Rack and Stack” in a live active medical environment. There are thirteen (C9410R) Network Ethernet Chassis that will be replacing the existing Network Ethernet Chassis and Switches. There are fifteen network 48 port Ethernet Network Switches (3750’s) that are to be replaced with five (C9410R) Ethernet Network Chassis. There are eight network 48 port Ethernet Network Switches (C6509-V-E) that are to be replaced with (C9410R) Ethernet Network Chassis. Detailed steps to be performed in each telecommunications room are included within. Generally these steps include, but not limited too. Removing Ethernet Switch to patch panel cabling harnesses called plug packs. Removing power to the network equipment. Removing the fiber connectivity from the network equipment to backbone network distribution. Remove the existing ground to the equipnment. Remove the existing network equipment. RE-installation of new University provided network equipment. Connection and reinstallation of power, fiber connectivity and grounding. The installation of a new contractor provided patch panel to network switch harnesses. Re-patching of all network ports to field equipment identically to the original connectivity. Additional support to resolve issues following events shall be provided by the contractor. Additional details to be included in the scope bellow. QuailficationsThe Vendor shall provide 4 experienced teams ( 2 Persons = Team) that have previous experience in installing and replacing Cisco network equipment in new and existing sites. Sites is defined as a location where at least 300 ports were patched to field devices. Experience patching network equipment and developing port to cable documentation.C-7 Licensed contractor Minimum of 3 years experience installing network equipment in a similar environment.RequirementsVendor Staff shall be required to complete training class developed by vendor and approved by the Universities Representative. This Training class shall include a complete mock up of the scenario(s) to be completed in the actual replacement of equipment outlined later in this document. These costs are to be wholey inclusive in this RFQ pricing. There will be a general schedule to follow based on customer impact and can change at any time. Flexibility is a requirement due to the fact that this has direct impact on patient care there for at any time scheduled and locations can change at any time. Cost incurred as a result of patient care needed shall be full responsibility of the vendor and included in your cost proposal. There will be a mandatory walk to go over scope of work and layouts for all TRs. Contractor shall assume all responsibility of tasks unless noted otherwise. Insurance liability shall include accidental damage to existing University cable and property. If so, UC will submit a written Amendment to Supplier describing the changes in appropriate detail. If an Amendment does not require Supplier to incur any additional material costs or expenses, then Supplier will make the modification within ten (10) business days of Supplier’s receipt of UC’s Amendment. If an Amendment does require that Supplier incur additional material costs or expenses, then Supplier in good faith will provide UC with a written, high level, non-binding assessment of the costs and expenses and the time required to perform the modifications required by the Amendment, within ten (10) business days of Supplier’s receipt of UC’s Amendment. UC will notify Supplier in writing within ten (10) business days after receipt of Supplier’s response to the Amendment as to whether UC wishes Supplier to implement the Amendment based on the response. UC will compensate Supplier for implementation of an Amendment in accordance with the terms and conditions of the relevant Amendment and Supplier’s response to the Amendment, if any. Supplier’s implementation of an Amendment will not delay the performance of Services and/or the delivery of deliverables not reasonably affected by an Amendment. No Mandatory UseBecause there is no mandatory use policy at UC, nothing in this Statement of Work will be construed to prevent UC from entering into similar agreements with any third parties including, without limitation, suppliers that may be in competition with Supplier.
