Job Objectives:

Design, develops, and implements an ostomy and wound clinical specialty program for Home Care clients. The purpose of the program is to improve outcomes for ostomy and wound related client issues. Provide direct client care and education to home care clients - ostomy and wound clients. Provide ostomy and wound care management consultation to staff. Develop a wound and ostomy Management Team to ensure clinical competency in the area of ostomy and wound care and to provide a resource team to home care.

Job Responsibilities:

I. Design, develops, and implements the agency Ostomy and Wound Care Programs

1. Assess the needs of ostomy and wound population served by the agency

2. Assess the needs of referral sources as they relate to ostomy/wound care.

3. Establish program goals and measure program progress on a regular basis

4. Establish and implement CQI plans for wound and ostomy

5. Establish/modify wound and otosmy management protocols

6. Develop/modify patient education materials in conjunction with Staff Education

7. Complete field supervisory visits for team members

8. Participate in the performance appraisals of team members

9. Function as a wound and ostomy resource person for field staff and managers.

II. Participate in the development of ostomy and wound related clinical pathways

1. Establish standards for each clinical pathway based on current clinical knowledge and in congruence with affiliates as appropriate

2. Maintain compliance with regulatory, agency, and community standards

III. Provide direct patient care in compliance with regulatory and professional standards and productivity expectations.

IV. Provide education regarding wound and ostomy for home care staff.

1. Assess home care learning needs.

2. Provide in-services to meet the assessed needs, including equipment use if appropriate.

3. Evaluate the effectiveness of the teaching/learning process with measurable outcomes

4. Maintains current resources in the library in conjunction with Staff Education.

V. Participate in organizational quality improvement activities.

1. Identify and implement quality improvement activities for the Wound and Ostomy Care Program.

VI. Develop and implement educational strategies to inform affiliates and other community health care providers of the capabilities and services of the Ostomy and Wound Care Management Team.

1. Establish a role for the agency’s Ostomy and Wound Disease Management Program.

VI. Support the Customer Service program by promoting a responsive, knowledgeable, and accessible approach to all customer needs.

VII. Participate in ViaHealth community outreach activities.

VIII. May be responsible to complete specimen collections in the field in accordance with all-appropriate policies and procedures.

IX. Age Specific Competency – Providing care and service appropriate to the ages of those served. This includes:

A. Based on assignment participates in age specific competency training within a specified time frame and demonstrates the ability to obtain information and assess the patient needs for the relevant age group(s).

B. Apply the knowledge of growth and development stages in direct patient interactions as observed by the supervisor for the relevant age group(s).

C. Understand the range of treatment and/or services needed by the different age groups served as observed by the supervisor.

VII. Perform other duties as assigned.

Preparation and Training:

MS in Nursing. License and current registration as a Registered Professional Nurse in NYS and certification by wound, ostomy, and continence Nurses society.

Experience:

Minimum 5-10 years nursing experience. 2-4 years in clinical program and program management.

Consequences of may result in patient harm.

Confidential Data:

Full and complete access to confidential patient medical record information, which requires integrity to safeguard the patient.

Level of Supervision:

Employee and manager develop objectives, define resources available, projects, and deadlines.

Mental/Visual Demand and Physical Effort:

Flow and amount of client referrals may produce a moderate level of mental fatigue.

Directs Work of Others and/or # of Employees Supervised:

Directly supervises no one. Participates in the evaluation of team members. Is responsible for the clinical competency of team members.

Environment:

Office environment with occasional home visits.
