II. Replace -- Section 3 -- Pricing Form

Remove Pricing Form - Page 3-18 to Page 3-29 dated Jun 14, 2018 in its entirety and replace with the attached -- Revised Pricing Form -- Page 3-18 to 3-29 dated Jun 28, 2018.

III. Revisions

I.

Secondary valves are deleted on long hydrant leads on Coldstream Ave., Glen Park Ave.,

and Hillmount Ave. Tender quantity for line item #12, 150mm gate valve and box has

been revised.

Site Specific Special Provision #27 -- Tree Protection Fencing -- SP27 is added to this

Contract.

3.

Tree Protection -- GN12855:

Delete the final Basis of Payment section and replace it with the following:

Basis of Payment

Payment for tree protection fencing shall be as detailed in 5P27. All other aspects of tree protection shall be incidental to all Work.

A new line item #81 A has been added for Tree Protection Fencing.

Temporary trench restoration for Bathurst Street, Dufferin Street, and Parkside Drive

shall consist of the following:

Line item #52, Remove 375 mm pipe, and line item #53, 375 mm reinforced concrete

pipe, CL 65D up to 3.0 m in depth have been moved to the Major Roads Part of the

Bid Form.

7.

New line items for Cold milling of asphalt pavement -- up to 50 mm depth have been

added to the Major Roads Part of the Bid Form.

8.

New line items for Superpave 12.5, Traffic Category D. PG 64-28 have been added to

the Major Roads Part of the Bid Form.

Contingency is ten percent (10%) of Bid amount as per revised Pricing Form attached to

this addendum.

IV. Questions and Answers

Q I: I was looking at the above City of Toronto Tender 166-2018 and I cannot seem to find

the amount of the percentage for the Contingency amount as the quantity is N/A on the item. If you could please let me know that would be great.

Al: Please see Revision # 9 above and the attached updated Pricing Form.
