A holistic care approach is used to promote and improve health and wellbeing. Service provision includes comprehensive nursing assessment, care planning, service coordination and monitoring within a CHN and a social model of health framework. This service is predominantly outreach based and care is provided in a range of community settings including private homes, public housing, supported residential services (SRS’s) and other settings as required.
