MEDICAL REPORT TO PARENTS (PINK EYE)
MEDICAL REPORT TO PARENTS (PINK EYE)
Date:____________Student’s Name:______________________________________________
Your child was seen in the XXX SBHC today by:
❑ XXX, Family Nurse Practitioner / Physician Assistant
❑ XXX, Nurse / Medical Assistant
❑ Other:
Your child presented with symptoms and a clinical exam that are consistent with conjunctivitis (pink eye). We suggest the following:
❑ Your child was seen by the Wellness Center nurse who suspects that your child has pink eye. You need to schedule an appointment with the provider at the Wellness Center or your child’s regular provider to confirm this diagnosis and to obtain treatment. It is school policy that your child should not be in school with untreated pink eye, so you need to seek care as quickly as possible.
❑ Your child was seen by the nurse practitioner who has confirmed a diagnosis of pink eye. A prescription is enclosed for an eye drop. You should start this immediately and use as instructed for one week.
❑ Other:
Additional instructions to prevent the spread of infection from one eye to another or to other people in your family:
❑ Frequent and good handwashing are the key to preventing the spread of infection!
❑ Try to avoid touching or rubbing your eye(s), even though they are irritated.
❑ If you wipe drainage from your eye, wipe from the inside corner (next to your nose) to the outside corner. This decreases the chance of spreading the infection across your face to the other eye.
❑ Throw away a tissue after you wipe your eye so you don’t use it again. Put washclothes in the laundry after one use if you wash your face with them.
❑ Do NOT use eye makeup until the infection has completely cleared. Your current eye makeup may already be contaminated and you should consider throwing it away and using new makeup once the infection is cleared.
❑ Do NOT wear your contact lenses until the infection is cleared. If you have disposable lenses you should consider starting with a new pair of lenses once the infection has cleared. If you do not have disposable lenses, you should clean your contacts as instructed. You should contact your eye doctor if you have any questions about using your contact lenses.
❑ Do NOT use Visine just to make your eye “look” better. This is harmful to your eye.
❑ Clean surfaces (computer keyboards, telephones, etc) that family members will use.
❑ Your infection is considered contagious until you have used your eye drops for 24-48 hours. If we treat your child at this clinic, then we will provide a school excuse for this. If you seek treatment elsewhere, remember to ask your provider for a school excuse.
