INTRODUCTIONWe are pleased that you are pursuing OIS certification. The workshop that you are about to experience combines elements of adult learning theory, current practices in disseminating information, casual learning environments, and practical application. We make every attempt to provide an environment for a maximum learning experience for each and every participant.We understand that people have different styles of processing information, as do Instructors. We also understand that the general learning environment must include certain elements required for optimal participation. We are also aware that certain behaviors are not conducive to effective learning. Given these factors, we ask that Participants adhere to the following guidelines of OIS Workshops. proceduresBe respectful of each other. Even if there is disagreement with one another, the Instructor, or a principle of OIS, it is critical that those disagreements are expressed in a respectful manner.OIS and its principles are grounded in research into most promising practices, practical application of techniques, experience, and Oregon Administrative Rules (which are based upon Federal regulations). As a Participant, you are required to adhere to all of OIS.The following represents the core values held by OIS. By signing below, you are acknowledging that you have read, understand, and agree with these values.Everyone deserves a high quality of life.Some people need significant supports.Support is best provided with person-centered emphasis.Supports must maintain a person’s dignity.We are more alike the people we support than we are different.We need to feel supported to best support others.Challenging behavior can compromise quality of life.We will be together for the better part of 2 days. Participation is required. It enhances the learning experience and improves the dynamic of the group as well.We will make every attempt to accommodate the special needs of participants. We will need to be informed of these needs in advance (physical restrictions, learning disability, etc.). certificatesIt is at the Instructor’s discretion whether to certify a participant or not, based on the completed workshop activities and documents, and the Instructor’s resources to provide follow up and practice or additional instruction and supervision.An Instructor may deny certification in a given workshop due to the following: Failure to demonstrate competency in the O.I.S. workshop activitiesInability to complete all physical skills interventionsParticipants demonstrated attitude of unwillingness to follow the basic principles of O.I.S.Not participating in the workshop in its entiretyAlthough it is rare, your Instructor may ask Participants to leave the workshop. In the unlikely event that the Instructor is forced to remove a Participant from the workshop, they will do it privately and in as a respectful manner as is possible.you acknowledge with your signature that you may not teach, practice, or write any o.i.s. physical skills techniques into positive behavior support plans. failure to comply will result in decertification of your o.i.s. certificate. acknowledgementPrint NameSignature Date ................
