I hope this letter finds you happy and healthy. I am writing to inform you of an upcoming change in your health care. I am excited to announce that I will be moving my practice to Austin Regional Clinic's newest clinic ARC Buda, located at 3420 FM 967, Suite B-100, Buda, TX 78610.

November 10, 2017 will be my last day seeing patients at ARC Kyle Plum Creek.

Please disregard this letter if you are not a regular patient of mine. Your care at ARC will not change. It is very important to me as well as to ARC that you maintain continuous health care. For your convenience, ARC's electronic medical records system allows your medical record to be accessible from any ARC location.

I would be delighted if you continue your care with me at ARC Buda, but I understand if you would like to stay at ARC Kyle Plum Creek. I have complete trust that my fellow Family Medicine colleagues will take excellent care of your health. I recommend the following providers at ARC Kyle Plum Creek.
