Reevaluation

Review data to determine the nature and extent of the student's needs

Reevaluation

Based on the review, the team will determine if additional information is needed to:

- Develop the student's PLAAFP - Determine existence of an adverse impact - Determine whether the student continues to need special

education and related services

- Determine whether any additions or modifications to the special

education and/or related services are needed 5

Reevaluation

If the team determines that no additional assessments are necessary provide Written Notice:

- Explain the decision and reasons for that decision - Inform the parent/adult student of his or her right to request

assessments to determine eligibility and educational needs

If the team determines that additional assessments are necessary, within the Consent for Assessment:

- Develop an assessment plan - Provide Written Notice regarding the proposed assessments

Obtain written consent from parents/adult student to administer the needed assessments

Reevaluation

After reasonable attempts to gain written consent from the parent/adult student, the team can move forward with assessment without consent.

- Follow district guidelines for "reasonable." - Document the measures taken to obtain consent.

Reevaluation Eligibility Report

The Evaluation Team is required to prepare an Eligibility Report that details how the reviewed data demonstrates continued eligibility.

Eligibility Report must address each required eligibility component, including results from previous assessments if they are being used to determine eligibility.
