GOALS AND INSTRUCTIONS

Case selection:

The goal is improvement, through self-critical analysis of cases that could have been managed differently. It is not essential for mortality or major morbidity to be the final outcome, but the consequences of actions taken or choices made must have been very important to the patient, or must have been potentially very important, if ultimately avoided. If there isn’t at least one element of management that makes at least one provider at least a tiny bit uncomfortable, it’s not an appropriate case.

The primary targets for improvement should be intramural entities capable of being present or represented for the discussion. A series of blunders at an outside hospital, snatched from the jaws of death by miraculous care at CUMC, is not an appropriate case. Where intramural management is concerned, stunning virtuosity is assumed, and may approach but should not outweigh stunning deficiencies. There are other venues for presenting or publishing our triumphs.

Case selection is a privilege granted to Divisions who consistently select appropriate cases. The Department will select the cases for Divisions that are non-conforming, or prefer not to choose.

Presentation:

The responsible attending surgeon must be present. When possible, all involved attending surgeons and residents should be present. The presenter is strongly encouraged to invite guests who were importantly involved in the case.

Please share your PowerPoint with Dr. Arnell, Dr. Smith, and Radiology at least 24 hours in advance.

Begin by presenting the facts in chronological order, in about 10 minutes. The case should unfold for the audience as it would for someone being asked for help with an evolving catastrophe. The audience should be actively wondering what was going on and what the options were. The audience should not be passively imbibing the presenter’s defense of a first slide that summarizes the case followed by review of the literature.

The series of slides that presents the facts of the case should end with a blank slide, or the “improvements” slide. A “blank” slide can be truly blank, or can be any image of the presenter’s choice. The “improvements” slide should appear first without boxes checked. The presenter’s conclusions on which boxes to check should appear as a second slide, or as an animation of the first slide. Now and then the Chairman may send the presenter a specific “blank” and/or discussion slides. Please indulge him by including them.

When the blank or “improvements” slide appears, the moderator will ask the audience for questions and comments.

The presenter’s editorial opinions, and insights derived from review of the literature, are very valuable, but are not part of presentation of the facts. Ideally those embellishments will emerge during discussion. The presenter will be invited to show back-up slides and deliver commentary at the moderator’s discretion.
