Any validation of student’s age, residency, and other documentation may be conducted subsequent to admission and enrollment of the child. We cannot deny enrollment of a child whose birth certificate cannot be presented. We will allow parents or guardians 30 days to produce the required documentation after enrollment. After the 30 days have expired and documents are not secured, contact Rusty Saine, District Homeless and Student Enrollment Liaison, at 704-736-1017 ext. 30173.The requirements for admitting a child into a school are as follows:COMPLETE HOME LANGUAGE SURVEYEvery student must fill out Home Language Survey before proceeding with enrollment. If the child meets the requirements for ESL, as indicated by the completion of the Home Language Survey, contact the ESL Center at 704-736-1017 ext. 30190. The ESL Center will verify the primary language, administer the W-APT Test, and complete the registration process. PROOF OF IDENTITYVerify the identity of the person presenting the child for enrollment by using one of the following for documentation:_____ A. Driver’s License_____ B. State issued DMV Photo ID_____ C. Military ID_____ D. Passport_____ E. Green Card (Cannot be copied)If identification cannot be produced, contact Rusty Saine, District Homeless and Student Enrollment Liaison, at 704-736-1017 ext. 30173 with detailed information.PROOF OF STATUSBiological Parents must provide one of the following for documentation:_____ a. Certified Birth Certificate showing names of parents._____ b. Paternity Order signed by Judge showing names of parents._____ c. Custody Order signed by judge showing names of parents.Non-biological Legal Custodian or Legal Guardian must provide one of the following for documentation:_____ a. Custody Order signed by Judge showing the name(s) of legal custodian._____ b. Guardianship Order signed by Judge or Clerk of Court showing the name(s) of legal guardian._____ c. Order of Adoption signed by Judge or Clerk of Court showing the name(s) of adoptive parents._____ d. Written Verification from DSS showing that DSS has taken custody of the child OR written verification from DSS indicating that DSS has placed the child with a person who is not the biological parent, legal guardian, or legal custodian. The paperwork must be show the name(s) and address of person who has been given temporary placement of child._____ e. If none of the above can be presented, determine if “Affidavit B and/or Affidavit C” apply. If so, the person(s) presenting the child for enrollment must complete the affidavit(s). PROOF OF DOMICILE IN THE LINCOLN COUNTY SCHOOL DISTRICTTwo proofs of domicile are required for verification of residency documentation:_____ a. DMV issued Driver’s License showing name and address where child resides._____ b. State issued DMV Photo ID showing name and address where child resides._____ c. Utility bill such as power, water, cable, gas, etc. (NO CELL PHONE BILL) showing the name(s) and address where child resides._____ d. Real Property Tax bill showing the name(s) and address where the child resides._____ e. Lease Agreement showing name(s) and address where child resides (may be typed or handwritten). Lease must be signed by both the landlord and the leasee. Contact information for the landlord must be included._____ f. Mortgage Statement or Closing DocumentIf two proofs of domicile cannot be presented:If the address of the person, with whom the child is currently residing, is a temporary living arrangement or if this temporary living arrangement is due to a loss of housing, contact Rusty Saine, District Homeless and Student Enrollment Liaison, at 704-736-1017 ext. 30173 and continue with McKinney Vento Paperwork.If the child is an unaccompanied youth less than 18 years of age, contact Rusty Saine, District Homeless and Student Enrollment Liaison, at 704-736-1017 ext. 30173.If a nor b apply, parent will need to complete “Affidavit of Domicile” (Form 2) and “Affidavit of Witness” (Form 3). These forms must be notarized.If the child is receiving, has received, is eligible for, or in need of exceptional children services, contact the Exceptional Children’s Teacher at your school. If additional assistance is needed, contact Kim Davis, Director of Exceptional Children, at 704-736-1017 ext. 30128.Children enrolled in group or foster homes:If the child has been placed in a group or foster home by DSS AND the biological parent, legal custodian, nor legal guardian retains custody, the child is entitled to be enrolled in the district where the group or foster home is located. DSS must provide the name(s), address, and contact information of the individual who has authority and responsibility to make education decisions for the child.If the child has been placed in a group or foster home by the child’s biological parent, legal custodian, or legal guardian AND the biological parent, legal custodian, or legal guardian retains legal custody, the child is entitled to be enrolled in the district where the biological parent, legal custodian, or legal guardian resides. “Affidavit C” must be completed.Make note of any special services needed for the child.PLEASE NOTE: If any situations arise that you are unsure of pertaining to enrollment, contact Rusty Saine, District Homeless and Student Enrollment Liaison, at 704-736-1017 ext. 30173 before enrolling the child.
