My Info – My File Library

How can I attach a file to my Portfolio?

To upload a file (certificate) to an MLP Form, you must first have added that file to your ‘File Library’. If you are already within a form, you will need to Save As Draft that request, to first go and upload your document. Once the file is uploaded as a Personal File in your File Library, you can return or proceed to the form and you will be able to ‘select’ (check off) that newly-added document to attach it to the submission

How do I complete my PIP form?

You can click on the PIP form to start completing your 2013-14 PIP. After entering PIP data, click SAVE AS DRAFT. When you are ready to submit your PIP to the administrator, click SUBMIT.

What should I select as my purpose when completing a Conference Request Form?

The ‘purpose’ section on the bottom of the district’s forms (workshops listed in the ‘District Catalog’ and the out-of-district conference) allows staff to select multiple purposes for the professional development activity. If the staff member selects the 100 Hours box, the workshop will automatically appear on the staff member’s transcript. The 100 Hour Record Form is a place for teachers to list professional development activities which do not require prior approval; e.g., team planning time, student teachers, etc. You can select multiple purposes for your professional development.

Where can I find the 100 Hour Record Form?

The 100 Hour Record Form is under Fill-In Forms on the left side of the MLP screen. If you select 100 Hour Record Form under purpose when requesting an out-of-district conference or registering for an after-school professional development offering, the information will appear on the 100 Hour Record form. This is an interactive form that can be completed throughout the school year. After entering activities in the 100 Hour Record form, click SAVE AS DRAFT. Only click SUBMIT when you want to submit a final document to your building administrator or district staff member completing your Annual.

How do I print a certificate of completion?

Once your request is in a Completed status, you can click on the blue title of that activity, scroll down to the Actions section, and then you will see a link called ‘Print Certificate.’ The certificate is available for printing as a PDF only. There are two places where you can view Completed activities: MLP homepage after you log into the site and under My Info Tab, on the left side of My Portfolio.

Where do I find the Out-of-District Conference Report Form?

After a staff member attends an out-of-district workshop, they will receive an email reminder to complete the Conference Report Form. You will have 5 days after the conference to complete the Conference Report Form (listed in MLP as Evaluation). Professional development hours are provided to the staff member after completing the Conference Report Form.

When will my certificate be available after a district in-service workshop?

Your certificate will be available after your attendance is verified and you have completed the workshop evaluation form. You will have 5 days from the district in-service workshop date to complete the survey and then the attendance will be verified.

If the survey is not completed by the time the attendance is verified, professional development hours will not be awarded. If the workshop evaluation form is not completed, your MLP profile will not list earned professional development hours.
