Acting Secretary of State Robert Torres announced that 143 disciplinary actions have been taken by the Bureau of Professional and Occupational Affairs' 29 professional licensing boards, and the Division of Legislation and Notaries of the Bureau of Commission, Elections, and Legislation:

Philadelphia County Janice M. Woodcock, registration no. RA014293B, of Philadelphia, Philadelphia County, was ordered to pay a civil penalty in the amount of $6,000, because she engaged in the practice or offered to engage in the practice of architecture in the Commonwealth, or used the title of architect or used any sign, card or devise implying she was competent to engage in the practice of architecture in the Commonwealth when she failed to hold a currently valid certificate as required by the Board. PA Board of Barber Examiners

Allegheny County Timothy J. Doyle, license no. BL050232L, of Pittsburgh, Allegheny County, was suspended for failing to pay a previously imposed civil penalty. (05/02/18)

Marc Blackwell, of McKeesport, Allegheny County, was denied licensure as a barber based on his felony convictions. (05/02/18)

Chester County Anthony R. Skelton, license nos. BL052913 & BM093633, of West Chester, Chester County, was indefinitely suspended with said suspension to be immediately stayed in favor of probation, based upon his felony convictions. (05/02/18)

PA Board of Certified Real Estate Appraisers

Cumberland County Stephen C. Coyle, license no. LAT000220, of Camp Hill, Cumberland County, was ordered to pay a civil penalty in the amount of $250, because he failed to comply with the continuing education requirements to renew his licensed appraiser trainee certificate in that he was two (2) hours short of the minimum twentyeight (28) total hours of continuing education required for the renewal of his certificate. PA Board of Chiropractic

Crawford County Brian Allen Miller, D.C., license nos. DC006913L and AJ006913L, of Venango, Crawford County, had his license to practice as a chiropractor temporarily suspended, pending a hearing, on the grounds that his

PA Board of Cosmetology

Allegheny County Dana M Stein, license no. CO237051L, of Pittsburgh, Allegheny County, was suspended for a period of one (1) year, such suspension to be immediately stayed in favor of probation subject to the terms and conditions of the Board; and she was ordered to pay a civil penalty in the amount of $500, because she has been convicted of a felony. Sandra L Perelstine, license no. CO168279L, of Homestead, Allegheny County, was actively suspended for a period of one (1) year, with such suspension to be immediately stayed in favor of probation for a period of one (1) year, subject to the terms and conditions of the Board; and she was ordered to pay a civil penalty in the amount of $500, because she has been convicted of a felony. Before And After Hair Design, license no. CB125111, of Pittsburgh, Allegheny County, was ordered to cease and desist from employing unlicensed employees in the practice of cosmetology and/or barbering, from aiding and abetting unlicensed practice activity in the professions of cosmetology and/or barbering, and from failing to have an owner or designated licensee in charge present during inspection of its salon, because it operated as a cosmetology salon in a grossly incompetent and unethical manner by employing unlicensed employees, with each unlicensed employee observed representing separate counts; it failed to have an owner or designated licensee in charge present during inspection; and it failed to display at or near its main entrance a sign that is clearly visible indicating to the public that it is a cosmetology salon or a limited practice salon. Kim Hong Thi Vu, license no. CQ111892, of Pittsburgh, Allegheny County, was levied a civil penalty of $500 based on practicing on a lapsed or expired license. (05/10/18)

Bradford County Wonderful Nail Spa, license no. CY196724, of Sayre, Bradford County, was ordered to pay a civil penalty of $14,500, costs of investigation of $71.32, and will be suspended for one (1) year, with such suspension immediately stayed in favor of no less than one (1) year of probation. Wonderful Nail Spa Corp. operated as a nail technology salon in a grossly incompetent, dishonest, and unethical manner through the employ of unlicensed individuals, with each unlicensed employee representing a single count, through the possession and use of Methyl Methacrylate, with each bottle found representing a single count, the operation outside the scope of its license by providing massage services by an unlicensed individual, and through the aiding and abetting the unlicensed practice of massage therapy. Delaware County Melissa E Miles, license no. CO254875, of Broomall, Delaware County, was suspended for one (1) year, such suspension to be immediately stayed in favor of probation subject to the terms and conditions of the Board; she was ordered to pay a civil penalty in the amount of $500; and a public reprimand was placed on her permanent disciplinary record with the Board, because she has been convicted of a felony. Franklin County Studio II, license no. CB120090, of Greencastle, Franklin County, was levied a civil penalty of $500, based upon failure to maintain salon in a safe, orderly and sanitary condition and failure to have required equipment. (05/10/18)

Lackawanna County Annie Nail Salon, license no. CY196553, of Peckville, Lackawanna County, was suspended for a period of one (1) year, such suspension to be immediately stayed in favor of probation subject to the terms and conditions of the Board; they were ordered to cease and desist from employing unlicensed individuals and from the use of metal rasp tools in the practice of nail technology; the salon was ordered to pay a civil penalty in the amount of $3,650 and an assessment fee of $76.70 towards costs of investigation; and a public reprimand was placed on their permanent disciplinary record with the Board, because they operated as a nail technology salon in a grossly incompetent and unethical manner by employing unlicensed individuals; they operated as a nail technology salon in a grossly incompetent and unethical manner through the use of metal rasp tool in the practice of nail technology; the salon failed to have a closed storage area for soiled linen; they failed to have a closed storage area for clean linens; and the salon failed to have a first aid kit containing a blood spill kit and eyewash. Davi Nails No. 4276, license nos. CY196031 and CZ117665, of Taylor, Lackawanna County, was levied a civil penalty of $1600, based upon failure employment of unlicensed individuals and failure to have required equipment. (05/10/18)

Lancaster County Hair Salon at Meadow Ridge, license no. CB119529, of Willow Street, Lancaster County, was levied a civil penalty of $5000, for failure to maintain records of services rendered outside the salon. (05/10/18)

Lehigh County Brittney N. Conrad, of Bethlehem, Lehigh County, was assessed a $500 civil penalty based on practicing without a license. (05/10/18)

Luzerne County Jessica Malone, license no. CO266685, of Wyoming, Luzerne County, agreed to the permanent voluntary surrender of her license to practice as a cosmetologist in the Commonwealth of Pennsylvania, because she has been convicted of a felony. (09/10/18)

Colorful Nail And Spa, license no. CY196393, of Hazleton, Luzerne County, was ordered to cease and desist from possessing and using metal rasp and razor tools and shall further cease and desist from operating outside the scope of a nail technology salon, including esthetic and massage services, shall pay a civil penalty of $10,000 and costs of investigation of $222.74, and its license will be publicly reprimanded, because the salon failed to have the current licenses available on the premises, with each missing license representing a single count; had an incomplete first aid kit; failed to post a "No Smoking" sign; maintained an unlicensed esthetician salon; operated outside the scope of Respondent's license by offering massage services; failed to have one closed towel cabinet for clean linens, with each violating

representing a single count; possessed and used razor and metal rasp tools, with each tool being a separate count; possessed and used corn planer blades; failed to have adequate lavatories on the premises; and failed to conspicuously display consumer protection information. (9/10/18)

Montgomery County Anna K. Delzingaro, license no. CO268369, of Hatboro, Montgomery County, was suspended for one (1) year, with such suspension immediately stayed in favor of no less than one (1) year of probation, ordered to pay a civil penalty of $500, and her license will be publicly reprimanded, because she has been convicted of any felony. Northampton County Cali Nails, license no. CY196640, of Easton, Northampton County, was ordered to cease and desist in the possession and use of Methyl Methacrylate substances in the practice of nail technology; they were suspended for a period of one (1) year, such suspension to be immediately stayed in favor of probation for a period of one (1) year, subject to the Board's terms and conditions; they were ordered to pay a civil penalty in the amount of $1,500; and a public reprimand was placed on their permanent disciplinary record with the Board, because they operated as a nail technology salon in a grossly incompetent and unethical manner through the possession of Methyl Methacrylate substances in the practice of nail technology. Philadelphia County Rhonda Y Copeland, license no. CO231345L, of Philadelphia, Philadelphia County, was ordered to pay a civil penalty in the amount of $750, because she violated the law by practicing cosmetology without a current and active license. (09/10/18)

Magaly Spa, license no. CB122871, of Philadelphia, Philadelphia County, was ordered to pay a civil penalty in the amount of $2,000 and an assessment fee of $115.90 towards costs of investigation; and a public reprimand was placed on their permanent disciplinary record with the Board, because they operated as a cosmetology salon in a grossly incompetent, dishonest, and unethical manner by employing an unlicensed individual; and they operated without a manager or other designated licensee present. Washington County Shallyn Ann Bodnar, of McMurray, Washington County, was assessed a $500 civil penalty based on practicing without a license. (05/02/18)

Westmoreland County Lindsay M Halliday, license no. CQ111641, of Mt Pleasant, Westmoreland County, agreed to the permanent voluntary surrender of her license to practice as an esthetician in the Commonwealth of Pennsylvania, because she has been convicted of a felony. (09/10/18)

Europe Nails, license no. CY193912, of Greensburg, Westmoreland County, was ordered to pay a civil penalty in the amount of $1000, because it violated the law by using or maintaining any place for the practice of nail technology without a current and active license. (09/10/18)

Wyoming County Angel Ann Halchak, license no. CO234163L, of Tunkhannock, Wyoming County, was indefinitely suspended based on her felony conviction. (05/10/18)

Adams County Kristin Elizabeth Nelson Russo, license no. DS039489, of Gettysburg, Adams County, was ordered to pay a civil penalty of four thousand dollars, because she violated the Act which requires a dentist to possess a current permit issued by the Board before administering, or supervising the administration of general anesthesia, deep sedation, conscious sedation or nitrous oxide/oxygen analgesia in a dental office; and by engaging in unprofessional conduct. Montgomery County Jonathan William Theis, license no. DS038087, of Conshohocken, Montgomery County, had his license to practice as a dentist in the Commonwealth of Pennsylvania revoked, because he is unable to practice dentistry with reasonable skill and safety to patients by reason of illness, drunkenness, excessive use of controlled substances or as the result of any other mental or physical condition. Heather Elizabeth Clark, license no. DS031551L, of Glenside, Montgomery County, was levied a civil penalty of two thousand dollars and had a public reprimand placed on her record with the board, because she knowingly or negligently employed a dental hygienist whose license or certificate was not current or had been suspended or revoked. Northampton County Palmer J. Cotturo, license no. DS015065L and permit nos. DN015065A and DP000611, of Mount Bethel, Northampton County, permanently and voluntarily surrendered his license to practice dentistry because he engaged in unprofessional conduct. (09/14/18)

Out of State Chad C. Taylor, PA license no. DS036668, of Houston, Texas, had a public reprimand placed on his permanent disciplinary record with the Board and was required to pay a $3,000 civil penalty because he had a license to practice dentistry revoked, suspended or had other disciplinary action imposed or consented to by the proper licensing authority of another state, territory or country or had an application for license refused, revoked or suspended by the proper licensing authority of another state, territory, or country. Kelley Renee Hagen, PA license no. FD012861L, of Philadelphia, Philadelphia County, was revoked and assessed a $10,000 civil penalty, based upon engaging in gross incompetency, negligence and misconduct in carrying on the profession; aiding or assisting an unlicensed person in engaging in an act or practice for which a license is required; and aiding and abetting the unlicensed practice of funeral directing. Bucks County Amanda Herlihy, of Bensalem, Bucks County, was granted a license to practice and immediately placed on probation, based on crimes of moral turpitude. (02/28/18)

Montgomery County Greg A. Bolles, license no. MSG009724, of Gilbertsville, Montgomery County, was suspended for no less than three years retroactive to January 29, 2018 for violating a consent agreement entered into with the Commonwealth. (04/13/18)

Philadelphia County Christopher Ronald Timberlake, license no. MSG009039, of Philadelphia, Philadelphia County, was revoked, assessed a civil penalty of $20,000 and assessed a $354.42 cost of investigation fee based on conviction of multiple misdemeanors which are crimes of moral turpitude, and for violating the Board's regulation by engaging in sexual harassment, sexual impropriety, sexual violation, or sexual abuse. Warren County Douglas Carl Maloy, license no. MSG000317, of Warren, Warren County, voluntarily surrendered his license, because he had a license suspended, revoked or refused or received other disciplinary action by the proper licensing authority of another jurisdiction. (9/18/18)
