Butler County Thomas Charles Pfeifer, certificate and license no. CA016746L, of Mars, Butler County, was ordered to pay a civil penalty of $1,500 because he engaged in unprofessional conduct that brought the profession of public accounting into disrepute or lowered public esteem for the profession. Cumberland County Charles Joseph Henry, certificate and license no. CA039866L, of Mechanicsburg, Cumberland County, was ordered to pay a civil penalty of $2,000 and costs of investigation of $207.51 because he engaged in the practice of public accounting during a period in which he did not hold a current license. CO268115, of Pittsburgh, Allegheny County, was suspended for a period of one (1) year, such suspension to be immediately stayed in favor of probation subject to the Board's terms and conditions; she was ordered to pay a civil penalty in the amount of $500; and a public reprimand was placed on her permanent disciplinary record with the Board, because she violated the law by practicing cosmetology without a current and active license. Armstrong County Dawn R. Barclay, license no. CO209349L, of West Leechburg, Armstrong County, was suspended for a period of one (1) year, such suspension to be immediately stayed in favor of probation subject to the Board's terms and conditions; was ordered to pay a civil penalty in the amount of $500; and a public reprimand was placed on her permanent disciplinary record with the Board, because she has been convicted of a felony. CY197430, of Blandon, Berks County, was ordered to cease and desist from possessing Methyl Methacrylate substances; and it was ordered to pay civil penalty in the amount of $500 because it operated as a nail technology salon in a grossly incompetent and unethical manner through the possession of Methyl Methacrylate Substance. Bucks County Tracey M. Turchi, license no. CO232689L, of Quakertown, Bucks County, was suspended for a period of one (1) year, such suspension to be immediately stayed in favor of probation for a period of one (1) year, subject to the Board's terms and conditions; she was ordered to pay a civil penalty in the amount of $500; and a public reprimand was placed on her permanent disciplinary record with the Board, because she has been convicted of a felony. Lackawanna County Fancie Nail Spa, license no. CY197584, of South Abington Township, Lackawanna County, was suspended for a period of one (1) year, such suspension to be immediately stayed in favor of probation subject to the Board's terms and conditions; it was ordered to cease and desist using metal razor tools in the practice of nail technology; it was ordered to pay a civil penalty in the amount of $1,050 and an assessment fee of $26.98 towards costs of investigation; and a public reprimand was placed on its permanent disciplinary record with the Board, because it operated as a nail technology salon in a grossly

incompetent, dishonest and unethical manner through the use of metal razor tools; and it violated the Board's regulation in that the labeled first aid kit did not contain a blood spill kit and eyewash. (07/09/18)

Lawrence County Mary L. Turner, license no. CO256218, of New Castle, Lawrence County, was suspended for a period of one (1) year, such suspension to be immediately stayed in probation subject to the Board's terms and conditions; was ordered to pay a civil penalty in the amount of $1,000; and a public reprimand was placed on her permanent disciplinary record with the Board, because she employed an unlicensed individual; and she utilized a cosmetology salon for purposes other than cosmetology, namely the injection of Botox. Lehigh County Oriental Nail, license no. CY101263L, of Allentown, Lehigh County, was ordered to cease and desist possession and use of MMA in nail technology practice, and ordered to pay a civil penalty in the amount of $500 and an assessment fee of $75.08 towards costs of investigation, because they operated as a cosmetology salon in a grossly incompetent, dishonest, and unethical manner by possessing Methyl Methacrylate ("MMA"). Luzerne County Nail Art, license no. CY194701, of Hazleton, Luzerne County, was ordered to cease and desist from employing unlicensed individuals; it was actively suspended for a period of one (1) year, such suspension to be immediately stayed in favor of probation subject to the terms and conditions of the Board; it was ordered to pay a civil penalty in the amount of $2,000 and an assessment fee of $24.19 towards costs of investigation; and a public reprimand was placed upon its permanent disciplinary record with the Board, because it operated as a nail technology salon in a grossly incompetent and unethical manner through the employment of an unlicensed individual; and it aided and abetted unlicensed practice activity in the profession of cosmetology. Dawn M. Cinti, license no. CT015602L, of Plains, Luzerne County, was suspended for a period of one (1) year, such suspension to be immediately stayed in favor of probation subject to the Board's terms and conditions; she was ordered to pay a civil penalty in the amount of $1,000; and a public reprimand was placed on her permanent disciplinary record with the Board, because she continued to practice cosmetology in a place other than a licensed salon in violation of a Board Order. Monroe County Heaven On Earth Hair Boutique And Spa, license no. CB124855, of Stroudsburg, Monroe County, was ordered to cease and desist from employing unlicensed employees in the practice of cosmetology and/or barbering, from aiding and abetting unlicensed practice activity in the professions of cosmetology and/or barbering, from failing to have an owner or designated licensee in charge present during inspection of its salon, and shall further cease and desist from possessing and utilizing metal credo tools in the practice of cosmetology; and it was ordered to pay a civil penalty in the amount of $500 and an assessment fee of $35.37 towards costs of investigation, because it operated as a cosmetology salon in a grossly incompetent and unethical manner through the possession and use of a metal credo tools. Nail Trix, license no. CZ117009, of Stroudsburg, Monroe County, had their authorizations to practice the profession placed on probation for one (1) year, subject to the Board's terms and conditions; they were ordered to pay a civil penalty in the amount of $400 and an assessment fee of $62.95 towards costs of investigation; and a public reprimand was placed on their permanent disciplinary record with the Board,

because they failed to have an owner or designated licensee in charge present during inspection; failed to have availability of current licenses and permits on the premises; failed to properly sterilize equipment; and failed to maintain all areas of the salon in an orderly and sanitary condition. Philadelphia County 3rd Element Spa and Salon, license no. CB119610, of Philadelphia, Philadelphia County, was suspended for a period of one (1) year, such suspension to be immediately stayed in favor of probation subject to the terms and conditions of the Board; they were ordered to pay a civil penalty in the amount of $500; and a public reprimand was placed on their permanent disciplinary record with the Board, because they had a massage therapist as his own independent contractor renting space in the salon. Maria L. Reynoso, of Philadelphia, Philadelphia County, was ordered to cease and desist from performing any activity requiring a license to practice as a cosmetologist in the Commonwealth of Pennsylvania; and she was ordered to pay a civil penalty in the amount of $1,000 and an assessment fee of $63.67 towards costs of investigation, because she practiced cosmetology for compensation without the required license. York County Bellagio Nails LLC, license no. CY196575, of Manchester, York County, was ordered to cease and desist from employing unlicensed individuals in the practice of nail technology; it was suspended for a period of one (1) year to practice as a nail technology salon in the Commonwealth of Pennsylvania, such suspension shall be immediately stayed in favor of probation subject to the Board's terms and conditions; it was ordered to pay a civil penalty in the amount of $1,000 and an assessment fee of $90.74 towards costs of investigation; and a public reprimand was placed on its permanent disciplinary record with the Board, because it was grossly incompetent and practiced unethically in employing an unlicensed individuals. Berks County Elaine M. Hoppes-Goroshko, license no. DS026062L, of Womelsdorf, Berks County, was indefinitely suspended for engaging in unprofessional conduct. (03/27/18)

Lycoming County Charles M Burzynski, license no. DS019188L and anesthesia permit no. DA019188A, of Williamsport, Lycoming County, had his license to practice as a dentist in the Commonwealth Pennsylvania suspended for a period of no less than twelve months, with said suspension stayed in favor of probation; was levied a civil penalty of $1,875; and an assessment for the costs of investigation of $3,408.10, because he engaged in unprofessional conduct in the practice of dentistry by failing to adequately communicate a radiographic finding to his patient, the patient's parents or the referring dentist. Montgomery County Carolyn C. Alderfer, license no. DH008290L, of Telford, Montgomery County, was indefinitely suspended based upon pleading guilty to crimes of moral turpitude. (03/26/18)

Khosrow Azizi, license no. DS025908L, of Ambler, Montgomery County, was ordered to pay a civil penalty of $5,000 and an assessment for the costs of investigation of $607.60; to cease and desist from the practice of withholding patient records upon receipt of written request; to complete and submit an application for a fictitious name for his dental office, Advanced Orthodontic Care, to the board within ninety days; and to complete the process of legally changing his name to "Benjamin K. Azizi" in accordance with 54 Pa. C.S. 701 et seq. because he engaged in unprofessional conduct when he failed to furnish an exact copy of the patient's written dental record upon receipt of a written request from a patient, and practiced dentistry under a name other than that on his license and biennial renewal. PA Board of Professional Engineers, Land Surveyors, and Geologists

Bucks County Harish J. Bhanderi, license and registration no. PE044497E, of Newtown, Bucks County, paid a civil penalty of $1,000 and was placed on probation to run concurrent with the criminal probation imposed by the Superior Court of New Jersey, County of Mercer, Law Division, Criminal, subject to the Board's terms and conditions, because he was guilty of misconduct in the practice of engineering. Chester County Matthew Mesko, license and registration no. PE040660E, of West Chester, Chester County, was indefinitely suspended to practice as an engineer in the Commonwealth of Pennsylvania, because he is guilty of misconduct in the practice of engineering since disciplinary action was taken against a license held by him to practice engineering by the proper licensing authority of another state for conduct relating to the practice of engineering, which occurred on two occasions. PA Board of Funeral Directors

Montgomery County Stanley A. Szpindor, license no. FD010818L, of Norristown, Montgomery County, was ordered to permanently surrender his license, because he failed to comply with the continuing education requirements to renew his funeral director license in that he was four (4) hours short of the minimum six (6) total hours of continuing education required for the renewal of his license, and obtained or attempted to obtain a license by the practice of any fraud or deceit. York County Patrick Francis O'Brien Jr., license no. FD013945L, of York, York County, was ordered to pay a civil penalty of $1,000 and the costs of investigation of $202.61 because he engaged in gross incompetency, negligence, or misconduct in the carrying on of the profession. (7/05/18)

MD039454E, of Coraopolis, Allegheny County, had his license to practice as a medical physician and surgeon temporarily suspended, pending a hearing, on the grounds that his continued practice within the Commonwealth may be an immediate and clear danger to the public health and safety. Linda Zagorski Cline, license no. YM003813L, of Pittsburgh, Allegheny County, permanently surrendered her license, because she cannot practice as a respiratory therapist with reasonable skill and safety to patients by reason of illness, addiction to hallucinogenic, narcotic or other drugs or alcohol, or mental incompetence. (07/24/18)

Bucks County Frederick Reichle, license no. MD006646E, of Warrington, Bucks County, had his license to practice Medicine in the Commonwealth of Pennsylvania temporarily suspended, pending a hearing, on the grounds that his continued practice within the Commonwealth may be an immediate and clear danger to the public health and safety. Erie County Qaisar Usmani, of Erie, Erie County, was granted a license to practice medicine and surgery, which was immediately placed on probation based upon disciplinary action taken by another state. (03/15/18)

Luzerne County Thomas Madden, license no. YM013572, of Wilkes Barre, Luzerne County, had his license to practice as a respiratory therapist indefinitely suspended for no less than three years retroactive to February 6, 2018, for violating a previous Board order. (03/19/18)

Northumberland County Matthew Mark Godfrey, license no. RT004810, of Watsontown, Northumberland County, agreed to the permanent voluntary surrender of his license to practice as a medical athletic trainer in the Commonwealth of Pennsylvania, because he was convicted of a felony or a misdemeanor relating to a health profession or received probation without verdict, disposition in lieu of trial or an Accelerated Rehabilitative Disposition in the disposition of felony charges, in the courts of this Commonwealth, a Federal court or a court of any other state, territory or country; he has been convicted of a felony; and he has been convicted of a misdemeanor related to the practice of the profession. Philadelphia County V. Renia Bey, unlicensed, of Philadelphia, Philadelphia County, was issued a $10,000 civil penalty because she engaged in the practice of medicine without a license and held herself out as a medical doctor by use of the title. (03/26/18)
