Activities completed throughout the placement:

• Comprehensive Patient Care (includes documentation and patient education) – minimum of 16 patients

• Patient care discussions; debrief with regular, consistent and timely feedback

o Discuss and review student documentation (care plans/correspondence with other health care professionals (please cosign)

• Student to answer all drug information requests

• Participate in scope of practice as applicable to practice setting i.e. injections

Activities to be added to Placement Calendar:

• Learning Plan Assignment (see Pharm 426 Syllabus; Appendix 6)

• Clinical Judgment Activity x 3 : Student to write at least 1 summary to review with preceptor; additional 2 discussions written summary is preceptors discretion

• Student/Preceptor discussions: (Professionalism, Communicator, etc; see Pharm 426 Preceptor Quick Reference Guide or syllabus. Complete list is in student’s eClass.

• Health Care Inter-Professional Visits: information in Syllabus (pg 17) to send to health care professional; discuss prior to activity & debrief with preceptor after

• Health Awareness Clinic; public screening or educational clinic; can be in pharmacy or in community. (not substitute for Community Practice Project but can be linked to the project)

• Health Promotion Presentation: provide presentation to the public: schools, teachers, students, seniors groups. Should be completed in the community. (not substitute for Community Practice Project but can be linked to the project)

• Patient & Medication safety activity and discussion

• Student to provide overview of the library resources and search strategies for the U of A Library Database(s) now accessible to preceptors (optional)

Other activities (if applicable to your site):

• Students participating in the PBS Program: Seminar Course dates across each term (to be added pending Block): Time is 1200-1330 on select Monday or Wednesdays as indicated below:

▪ Block 1: September 3 (week 2), September 17 (week 4), October 17 (week 8)

▪ Block 2: November 5 (week 2), November 26 (week 6),

▪ Block 3: January 14 (week 2), January 28 (week 4), Feb 18 (week 7),

▪ Block 4: March 18 (week 3), April 8 (week 6)

o Students in Edmonton only: students may participate in 1 Skills Lab at the faculty across the placement. If students sign up for a lab, they are responsible to let the preceptor know which date they will be at the faculty.

o Mentoring Activity (fall term only): sites are asked to participate in the Professionalism/Patient Care Mentoring activity for first year students. Preceptors and students will be provided with information prior to the start of the placement.

o Students in Calgary only: students will be advised of opportunities to participate in inter-professional activities.
