Mentor Checklist for BTSP Teachers

Call teacher to verify information provided on the assignment sheet (address, phone number, email, site administrator, PD, etc.).

Instruct the teacher to complete and mail the EESLPD Change form to the EESLPD Office, if information has changed.

Set up the Coaching Commitment Conference.

September/October

Team Agreement (Virtual) Meeting and Orientation will be scheduled by the EESLPD Office. Notify the teacher

and site administrator of the team agreement meeting dates at least 2 weeks in advance. Verify that the teacher has received the

following electronic documents:

Team Agreement (LE/SPI or SPII)

Teacher Evaluation Schedule

Rubric for Evaluating NC Teachers

LE teachers only - Pre-Service Checklist

Teacher will email the signed Team Agreement form to the mentor. The mentor will verify that the teacher and site

administrator have signed the form.

Mentor will sign the Team Agreement form and email it to the evaluator

Post-Observation Conference for 1st Observation (face-to-face, phone or virtual platform)

Additional October mentor/mentee contact

Introduction to Foundation modules, Intermediate or Advanced Course classroom-based support

NCEES-Home Base support, as needed

Introduce teacher support resources on website

October/November

Begin or review Professional Development Plan (PDP) planning for teacher based on teacher’s self-assessment and comments from first observation

Review the teacher’s goals to ensure that they are relevant to his/her professional growth.

Discuss the Initial Professional Development Plan with the teacher

Teacher enters the Professional Development Plan into NCEES- Home Base

Write Comments (required) and Sign (required) the Initial Professional Development Plan in the NCEES- Home

Base System for teacher

Lateral Entry teachers only - support teachers through the Pre-Service checklist, including Guided Observation and Foundation Modules, Intermediate Course

Mid-year PDP Review – Check the teacher’s summary of how he/she has demonstrated progress towards meeting each

PDP goal (summary of implementation of the activities and expected outcomes). Goals should be listed

separately (example: Goal 1: Standard 3, element A; Goal 1:3A – summary/reflection and evidences, Goal 2: 4H –

summary/reflection and evidences)

Write Comments (required) and Sign (required) the Preliminary/Professional Development Plan in the NCEES- Home

Base System for teacher

Post-Observation Conference for 3rd Observation (face-to-face, phone, or virtual platform)

March

Professional Development Plan – End-of-Year Review- All Teachers no later than May 1st (face-to-face, phone, virtual platform).

Review the End-of-year PDP Review – Check the teacher’s summary of how goals were implemented and whether or not

they were met. Teacher will complete a summary of activities implemented and outcomes.

Goals should be listed separately (example: Goal 1: Standard 3, element A; Goal 1:3A – Goal 2: 4H –

summary/reflection and documentation artifacts)

Review the teacher’s evidences to support the summary of each goal.

Write Comments (required) and Sign (required) the Preliminary/Professional Development Plan in the NCEES- Home

Base System for teacher.

Attend Final Observation and Summative Evaluation Conference

Advise teachers to print out or download copy of the Teacher Summary Rating form to keep in his/her files.

Provisionally Licensed / Lateral Entry Only- Remind mentee to mail in Annual Professional Development Log, including

unofficial transcripts of coursework to EESLPD Office - on or before May 31st.
