Dear Parents and Students,

The course offerings for Session 1 of the After School Activity Program included in this brochure begin the week of September 25th and end mid/late November. To register for a course, please send a completed registration form and a check* made out to the Old Rochester Regional School District with your child

no later than Friday, SEPTEMBER 15th. Courses will be filled on a first come,

first served basis. To help with courses that fill to capacity, you may wish to indicate a second choice on the registration form. If no second choice is indicated and a course is full prior to receiving your accurate registration and check, then your registration and check will be returned.

Tuition-based, after school programming is intended to expand, enrich, and enhance student learning in diverse extracurricular subject areas, classes, and activities. Our goal is to expand children's horizons and enrich their personal lives by exposing them to a variety of elementary exploration. Exemplary behavior is expected. Anything less may result in student removal from the program after one warning.

Parents are expected to arrive at the BACK ENTRANCE to RMS, promptly at the dismissal time. Failure to do so will result in the removal of students from the program with a forfeiture of the course's tuition. The back door is the pick-up location and instructors need to see you face-to-face in order to ensure safe dismissal. If alternative pick-up arrangements are needed, a note to the office, course instructor, and program director is required. If a student is not able to attend an activity on a given day, a note is also required.

Students who are absent from school will not be allowed to participate in the After School Activity on the day(s) of absence. Tuition will not be refunded after the first day of the course for any reason. Tuition assistance due to financial hardship is available on a limited basis. Please contact the program director with your specific need, PRIOR TO REGISTRATION DEADLINES.

Please return completed forms (separate forms are required for each child and/or multiple courses), along with properly completed check(s)* (payable to ORR School District), to the program director. Be sure to include with your payment, the registration fee of $10 per family. This fee is due at the time of registration for each and every session, and should be included with your total payment. It is a single fee to be paid in addition to the course tuition, regardless of the number of children and/or courses enrolled per family.

* Checks may take 3-4 weeks for administrative processing before appearing on your bank statement.

LIABILITY WAIVER and RELEASE: I understand that participation in voluntary after school activities, depending on the nature of the activity, may include risk of injury that can range in severity from minor to disabling to even death. Although serious injuries are not common in supervised programs, it is impossible to eliminate the risk. I/we understand that my child's participation in the School District's after school programs is voluntary and that my child and I/we are free to choose not to participate. I/we consent to my child's participation in any of the approved activities listed above I understand that the Old Rochester Regional School District, the schools of Marion, Mattapoisett and Rochester, their employees, officers and agents will not be liable for personal injuries and/or property damage as a result of my child's participation in such programs or activities. I/We, on behalf of myself and my minor child, agree to release, hold harmless and indemnify the Old Rochester Regional School District, the schools of Marion, Mattapoisett and Rochester, their employees, officers and agents, from any loss, cost, damage and/or expense of any nature, including all attorneys' fees and costs which I or my child may have resulting, either directly or indirectly, from my child's participation in voluntary extracurricular programs or activities. I/We give permission for our son/daughter to participate in all school activities, and do forever release the Old Rochester Regional School District, the schools of Marion, Mattapoisett and Rochester, their staff, volunteers and agents from any and all actions, all known and unknown personal injuries or property damage of said minor arising out of said activities, and also all claims or right of action for damages which said minor has or hereafter may acquire. I also have made arrangements to secure timely pick up of my child at the conclusion of each scheduled program, event, meeting, or practice. I also understand that the Late Fee policy may apply. In the event of an emergency, I hereby certify that I am the parent/lawful guardian of the above named student, and grant to Old Rochester Regional School District, the schools of Marion, Mattapoisett and Rochester, their staff, volunteers and agents full authority to take whatever action they may consider appropriate under the circumstances involved regarding the health and safety of my child and authorize them to obtain emergency medical or dental services for my child, if necessary, at my expense. By signing this Agreement, I/we acknowledge that we have read and understand this document and accept the risk and responsibility of participation in voluntary after school activities.

MINECRAFT CLUB Calling all "Gamers"!!! Share your creative technological interests with like-

minded "RMS Crafters" as students use iMacs to explore the ever-expanding worlds of this popular (more than 30 million players worldwide) online, interactive adventure game. Students will be guided through the Minecraft experience as an active member of our social-gaming community. Expert discussions will go beyond simple building and battling strategies to include: gaming etiquette, creative variety, survival tips/techniques, and the use of "mods". LIMITED TO FIRST 8 STUDENTS! GRADE LEVELS: 3-6 DAY: MONDAYS 9/25, 10/2, 10/23, 10/30, 11/6, 11/13, 11/20, 11/27 TIME: After school to 4:15pm INSTRUCTOR: J. Woodward and K. Woodward TUITION: $70 (Students must have their own Mojang account prior to the first day of class. The

Minecraft Mac/PC version must be purchased at and is needed prior to enrolling.) Usernames and passwords are required for each session.)

GREAT OUTDOORS If you like playing games, having fun, and making friends, the Great Outdoors is

the place for you! We'll explore the woods, sandpit, and bog behind school; build forts and learn about ropes; and of course, end it all off with our traditional S'MORs party! GRADE LEVELS: K-6 DAY: MONDAYS 9/25, 10/2, 10/23, 10/30, 11/6, 11/13, 11/20 Course is offered on multiple days, so BE SURE TO INDICATE MONDAY ON FORM! TIME: After school to 4:10PM INSTRUCTOR: T. Kinsky TUITION: $57 (includes $1 materials fee)

HOMEWORK CLUB Students will have the opportunity to complete homework in school under the

guidance of 2 teachers. This will help students avoid the stress of doing homework at home, and give them the extra support they may need to complete their assignments. GRADE LEVELS: 4-6 DAY: MONDAYS 9/25, 10/2, 10/23, 10/30, 11/6, 11/13, 11/20, 11/27 Course is offered on multiple days, so BE SURE TO INDICATE MONDAY ON FORM! TIME: After school to 4pm INSTRUCTOR: K. DellaCioppa and K. Lefebvre TUITION: $55 (Students may enroll in both M and TR sessions for $110. Write BOTH on reg. form)

INDOOR FLOOR HOCKEY Students will learn the basics of floor hockey and teamwork. NO experience is
