Instructions: This declaration must be used in lieu of the “Declaration of Originality” when one or both of the following apply:

1. the thesis includes material based on jointly authored research (published or unpublished) – complete section I;

and/or

2. the thesis incorporates the text of (or substantial parts from) one or more papers that you have published or submitted for publication – complete section II.

The text within the declaration must not be altered except for the wording of those parts in sections I and II in square brackets [ ] where you are asked to provide specific information regarding co-authors and publications.
