Weekend Clinical Liaison ? Pioneer Valley

Job Description: This position is primarily responsible for referral source and census development for Berkshire Healthcare facilities in the Pioneer Valley area, including the development of effective business partnerships with facility based teams and key accounts creating partnerships that create and sustain referral results.

Develops and maintains an account book of business focused on acute care and some community based healthcare agencies prioritizing effective business relationship management by providing exceptional customer service that sustains or grows referral results or identifies and develops new account relationships

Performs the physical and psychosocial assessments for patients referred including completion of PASRR's, and assures the completion of Levelings, OBRA's or other pre-admission requirements linked to admission approvals and reimbursement

Accepts patients screened as described above, in the field as indicated in the Clinical Grid based on each facility's competencies and effectively communicates with facility teams to present, educate, review, and share pertinent information to determine placement and/or assure for a successful admission

Monitors and supports screening for patient bed holds and helps facility staff stay apprised of any changes along with transition back to the facility

Coordinates referrals within Berkshire Healthcare based on geographical preference, specialized program, or bed availability

In partnership with the facility teams and in support of the Sales and Marketing plans, develops and maintains a 90 day plan for professional events that meet the needs of the referral sources, allows for team presence, and supports the ongoing relationship development goals of the accounts

Works collaboratively and cohesively with the various teams including facility based and Liaison team to understand 5 Star and Service mix goals, and assist with idea's and plans in support of the common objectives

Understands and works in support of the organization's mission, vision and values, always conveying a positive image of Berkshire Healthcare and it's facilities to all audiences

Fulfill other duties assigned by the Director of Marketing and/or Vice President of Business Development and Marketing

Effectively and flexibly manages time with minimal supervision. Possesses strong selling and communication skills actively listening, presenting solutions, overcoming objections and

closing the sales with referral sources, referral influencers and potential customers Maintains and effectively uses the sales and marketing software for setting up referrals, following up and

tracking. Proficiently uses reports from the program to trend outcomes and to develop strategies for relationship management. Submits daily, monthly and semi-annual reports identifying key trends and solutions to the Director of Marketing and Travel throughout the region required Position will be every weekend and Per Diem, as needed. Qualifications Successful experience of 2-5 years in a healthcare setting or healthcare sales setting, preferably LTC related BS or MSW Social Work, LPN, RN or BSN, with 2 years working experience in long term care Current license as RN, BSN, LPN or Social Worker required Experience in sales or business development, basic understanding of commercial insurances, Medicare and Medicaid reimbursement. Position Title: Child-Specific Social Worker Program: Intensive Foster Care (IFC) Reports To: IFC Program Supervisor Position Location: Northampton

Summary:

CSR's Intensive Foster Care Program works to place children in stable, loving, nurturing foster homes. Our goal is to find both short-term and long-term placements for at-risk children and youth with a broad range of emotional, physical and developmental special needs. Reunification with the child's biological family is our goal. When that is not feasible, care providers and staff work together to formulate an alternate plan for permanency such as kinship care, guardianship, or adoption.

Responsibilities:

Weekly face to face contact with all clients on your caseload, specifically: Ensuring proper medication administration Collecting encounter forms from medical and dental visits Establishing rapport with your client and assisting to build a relationship between your client and the foster family Trauma Informed Care Bedroom/house checks PAYA (with clients 14+) Will be assigned a duty day once per week Obtain and maintain Social Work/Mental Health License Foster Care Reviews School Meetings Psychiatrist, therapy, medical and dental appointments as needed

Other Expectations: Weekly participation in supervision and unit meetings; Monthly participation in department meetings rotating

between Roxbury and Tewksbury offices Professionalism and a positive attitude across settings Appropriate boundaries and work ethic

Educational/Experience Requirements: 1. Master's level or bachelor's level; in social work or a related field 2. meetings outside of business hours 4. Must have excellent writing and communication skills 5. Must have typing skills and experience with Microsoft Office 6. Must be willing to work in collaboration with the child's entire Treatment Team, including: DCF social workers,

Service Providers (therapists, physiatrists), biological families, school personnel etc. towards the goals of the child's Service Plan 7. Knowledge of referral coordination to community and private/public resources a plus 8. Experience in navigating any of the child-serving systems and experience advocating for children who are involved with behavioral health systems
