Saving/Renaming Files Self-Evaluation Form

Save a copy of the form Saving forms is slightly different depending on the browser you're using. In Internet Explorer, click the disk icon. In Chrome, click the download arrow. You should see the Save As menu. If you see a "Restrictions" error message, click OK to clear it. (Click OK more than once if needed.)

In the Save As menu, navigate to where you'll be saving (and working in) the file, such as a flash drive or your H drive. It's a good idea to have a folder specifically for your performance evaluations files. Save the copy of your form here.

ALERT: Save fillable PDF forms to your computer, flash drive or

H drive first. Fill the saved form only. Do not fill the website view of these forms.

Rename the form with nurse's name and year

Rename the form to include both: the nurse's name the year

Example: self-evaluation-Jane-Doe-2020.pdf

The file extension, ".pdf" will be automatically added to the end of the file name when you click the Save button.
