Everyone benefits when schools and community organizat ions work together to support learning. With the permissio n of the lega l guardian, community partners can access student- level demog raphic, discipline and academic data. They use data to track and strengt hen student performance and demonstrate the impact of their program. If you agree to allow our Commun ity Partners to support your student's progress, select t he blue Parent FERPA Consent Form . You will need to su bmit mu lt iple forms if your child partici pates in more than one program.

Family Educational Rights and Privacy Act (FERPA) Parents must sign a FERPA consent form each year allowing our community partners access to individual student data such as academic progress, demographics, assessments, teacher observations and student actions. FERPA defines education records as "records, files, documents, and other materials" that are "maintained by an educational agency or institution, or by a person acting for such agency or institution."
