Church of the Nativity Family MinistryMedical Care/Photo Release Statement:I, as parent or guardian of my son(s)/daughter(s) (hereinafter "child"), BYCLICKING “SUBMIT” ON THE REGISTRATION FORM, do hereby agree to allowmy child to participate in small groups program at the Church of the Nativity. Iagree to RELEASE AND HOLD HARMLESS AND INDEMNIFY Church of theNativity, the Archdiocese of Baltimore, the Roman Catholic Bishop of Baltimoreand his successors, a Corporate Sole, and all their agents, servants, andemployees from any liability, claims, demands, and causes of action arising outof or relating to any loss, damage, or injury sustained in connection with orarising out of my child’s participation in Small Groups. I also hereby grant Churchof the Nativity the right to use any photographs or videotape of my child, withoutidentification of my child by name, in publications, websites or other materialsproduced from time to time by Church of the Nativity. Further, I hereby grantpermission to any staff person to obtain medical care from a licensed physician,hospital, or medical clinic for my child in the event that I cannot be reached.Family Life Curriculum Agreement:I understand that by enrolling my child in one of the faith formation programs atNativity, my child will receive an integral part of our faith formation curriculum -the Catechesis for Family Life. This age-appropriate program about Christianliving, chastity, character formation, and safe environment training promotescommunication between you and your child. You are encouraged to review theprogram materials that the small group leader will be using, as well as thematerials you will receive for home discussion. After examining the program, ifyou have any questions or concerns about your child participating in thisprogram, please contact Maggie German (Director of Children's Ministry) or ChrisWesley (Director of Student Ministry). ................
