The fourth annual Get Healthy Know Your Numbers wellness screening period will be held September 1 to December 31, 2016. Wellness screenings

are offered to employees enrolled in PEBTF medical, prescription drug and/or supplemental benefits. Your covered spouse/domestic partner is not

required to complete a Know Your Numbers wellness screening in 2016.

The Know Your Numbers wellness screening looks at four measurements: ? Cholesterol ? Blood glucose (sugar) ? Blood pressure

Requirement for Get Healthy Program: Employee must complete a wellness screening by December 31, 2016 to be considered participating in the Get Healthy Program for July 1, 2017 ? June 30, 2018

? Height and weight to calculate Body Mass Index (BMI)

These four measurements could save your life ? especially if you have a reading that is beyond the healthy range, such as extremely high blood pressure.

There are three convenient ways to obtain your wellness screening: ? Onsite screening at select commonwealth worksites* ? Quest Diagnostics? Patient Service Center ? Your doctor's office (you must fax a completed Physician Results Form to Quest Diagnostics by December 31, 2016)

Registration opens August 1, 2016. Register online to make an appointment at an onsite event, Patient Service Center or to print a Physician Results Form. Register early for a wellness screening at an onsite event to make sure you get your preferred location and time. Registration for an onsite event will close on the Tuesday two weeks prior to the event.

*NOTE: State correctional institutions (SCIs) are open only to employees who work at that facility. DO NOT register for an event at an SCI unless you work there.

Wellness Screenings Benefit Members Participating in the Get Healthy Know Your Numbers wellness screening is voluntary. The 2016 wellness screenings are offered to employees enrolled in PEBTF benefits. The wellness screening gives you valuable health information so you know where you are doing well and areas that may need improvement. You should provide your doctor with a copy of your wellness screening results so you can discuss any follow-up that may be needed.

Why is the Know Your Numbers Wellness Screening Important? The wellness screening looks at four health measurements that could save your life ? cholesterol, blood glucose (sugar), blood pressure and BMI. Almost half of all Americans suffer from conditions such as diabetes, high blood pressure and high cholesterol, which wellness screenings help identify. More than 15% of people have one of these conditions and are unaware of it. A wellness screening can detect if you have any of these health risks so that you can work with your doctor to maintain or improve your health. The wellness screening is a way for you to find out how you are doing and to compare the results to previous wellness screenings.

Your Confidentiality is Assured All information in the wellness screening is personal and confidential as protected by federal law. Your results will not be read out loud at the onsite event. The reviewer will point to the results. The PEBTF will not have access to your individual results but will use aggregate data to develop future wellness programs. Quest Diagnostics values and understands that your privacy is most important and steps have been taken to assure confidentiality.

About Quest Diagnostics? Quest Diagnostics offers its Health & Wellness program to employees of both small and large companies. Quest Diagnostics began providing wellness screenings in 2002 and performed 1.5 million screenings this past year. This is the fourth year that Quest Diagnostics has performed wellness screenings for PEBTF members.

You may have a $15 PCP office visit copay. The

blood test will be billed to your medical plan. Your

doctor should screen for cholesterol, including HDL,

and glucose only. If you are enrolled in prescription

drug or supplemental benefits only, you may incur

costs under your non-PEBTF medical plan. CDHP

and Bronze Plan members will have the cost of the

office visit and blood test applied to their deductible.

Schedule directly with your doctor. Testing must be done between January 1, 2016 and December 31, 2016 so if you recently had a blood test, your doctor may submit those results.

Employees must take leave to visit the doctor.

How to Prepare for Your Screening

It is not necessary that you fast.

However, your results will be more accurate if you do not have any food or drink except water for 9 to 12 hours prior to your blood draw. Remember to drink plenty of water and take all medications as prescribed by your doctor.

How Do I Make an Appointment

Follow the registration instructions on the next page on or after August 1. You should register in advance, to guarantee an appointment.

When do I Get My Results /How do I Confirm that My Results were Received

Immediately at the event and online in 7 to 10 days.

Follow the registration instructions on the next page on or after August 1. You will then be able to choose an appointment date and time. You may register as a walk-in, however walk-ins are accepted based upon availability and you may have a wait time of up to 20 minutes.

Complete the online registration to print a Physician Results Form. It will populate with your demographic information. Schedule an appointment with your doctor well in advance of December 31, 2016. Provide the Physician Results Form to your doctor. Your doctor must complete the Physician Results Form in its entirety. You must fax the form to Quest Diagnostics by the deadline. Online in 3 to 5 days; printed copy mailed within 2 to 3 weeks. This is verification that your results were received by Quest Diagnostics and recorded.

How Do I Register for a Wellness Screening?

Registration opens August 1, 2016 for appointments beginning September 1 To register for an onsite event, make an appointment for a Quest Diagnostics Patient Service Center or print a Physician Results Form:

Register Online: 1. Go to on or after August 1. 2. Click on the Get Healthy logo on the left side of the home page. 3. Follow the instructions on the website to register for a wellness screening. 4. Returning users may log in with the username and password created last year. If needed, use the Forgot username or password? link to retrieve that information. 5. If you have not created a username and password, register as a new user. Click on the "Register Now" link in the "Sign Up Now" box. Enter registration key, PEBTF. The registration key is not case sensitive. Create an account by entering your first name, last name, date of birth and last 4 digits of your Social Security number. An email address is required for account set up.* 6. Schedule your wellness screening at an onsite event, a Quest Diagnostics Patient Service Center or print a Physician Results Form to take to your doctor's office. 7. After your screening, you will receive an email notice when results are available to view online. You will be able to compare your results to previous years' wellness screenings. Your printed report should arrive in two to three weeks (Patient Service Center and Physician Results Form only). Your results are proof that your wellness screening was recorded by Quest Diagnostics.
