On the dates listed above we sent you “Reminder Notices” requesting that you schedule your required Annual Recertification interview with Management. You have not responded and scheduled your interview.

If you do not contact the Management Office immediately and complete your Annual Recertification interview by

______________________________ 10th, 20_____________ (the month that is 60-days prior to the recertification date), your

rent will be raised to $ __________ (Market rate) effective _______________________1, 20 _________ (recertification date).

Paragraph 15 of your Lease Agreement authorizes us to implement such an increase in rent without sending you any additional notice.

If you do not contact the Management Office and supply the required information by the above listed date, we will be free to terminate your assistance, give your assistance (Section 8) to another resident and to charge you the market rate rent for the apartment.

Please do not make it necessary for us take this action and give your housing assistance to another household. Please call the Management Office at ____________________________ immediately to set up an appointment to discuss the proposed removal of your subsidy (Section 8 assistance).
