The rate of fall-related emergency department visits and mortalities is greatest among seniors (65 years old and over). Falls can result in chronic pain, a fear of falling, reduced mobility, loss of independence and even death. The good news is that most falls can be prevented. The first step to avoiding falls is to understand what causes them. These are called risk factors. Fall prevention takes an entire community. We all play an important role in helping older adults and seniors to age well and stay independent, healthy and strong. Even if we need to sit, it’s important to incorporate movement and take standing breaks. More and more research indicates that to age well and prevent/manage chronic disease, we need to sit less, move well and move more. VON SMART ProgramsSMART Rest & Retirement Homes44259513970VON facilitates group exercise and fall prevention education at various rest & retirement homes throughout the Erie St. Clair region. Classes are facilitated by a certified senior fitness instructor who will modify the exercises to accommodate most levels of mobility/ability.SMART In-Home One-to-One52641567310For those individuals who find it challenging to get to a community exercise program, have recently finished physiotherapy or other rehabilitation program, VON offers FREE In-home one-to-one exercise. A certified volunteer instructor will guide the participant through 15 gentle yet progressive exercises.SMART Community Groups57835664135A variety of FREE SMART exercise classes are made available to community members 55+. Specialty classes are also available for individuals living with Parkinson’s, MS and Acquired Brain Injury/Stroke. For more information visit: vonsmart.ca41SMART In-Home ProgramThe VON SMART In-Home Exercise and Fall Prevention Program is geared to older adults/seniors who are experiencing barriers to participation in traditional exercise groups due to low mobility, lack of transportation and poor health. This free 12 week program is led by a certified SMART volunteer who works one-to-one with an individual in their home. Sessions last approximately one hour. SMART In-Home consists of 15 simple, yet progressive exercises that are functionally focused to improve strength, balance and mobility. Posture, breathing and mindful awareness are also incorporated which may help individuals move well and move safely through activities of daily living.Individuals who may benefit from SMART In-Home should be:Be willing to try out the VON SMART In-Home Exercise Program Be able to follow instructions and recall the exercises (i.e. cognitively intact, as determined by physician diagnosis)Be ambulatory, with or without a walking aideNot be receiving ongoing professional rehabilitation services (i.e. occupational therapy or physical therapy) Possible exclusion guidelines:Individuals with the following characteristics may not be suitable:Any unstable medical conditions (angina, hypo-tension, uncontrolled diabetes, recent fracture or stroke)Extreme shortness of breathSuffering from uncontrolled painSMART In-Home Program continued…-762002038350Holidays, Inclement weather, Class cancellationsUnless otherwise agreed upon by the VON SMART volunteer and matched participant, the VON SMART program does not run on weekends or holidays.Sessions will be cancelled during periods of inclement weather i.e. ice/snowstorm.Sessions are cancelled for any flu/influenza like symptoms.If a session needs to be cancelled on the part of the VON SMART Instructor, the client will be notified in as timely manner as possible. SMART Education & Training Opportunities16298266488VON provides community members, interested in volunteering with the SMART Program, the opportunity to acquire SFI (Senior Fitness Instructor) and/or VON SMART Certification at no cost.
