Resolution authorizing contract extensions for Senior Options Medical Transportation (Lift and Non-Lift) Services (Office on Aging) (Estimated annual contract amount: $6,000,000.00)

WHEREAS, thirty-two (32) agencies were awarded contracts for Medical Transportation (Lift and Non-Lift) Services on September 13, 2011 under Res. No. 0654-11(ITB# 2011-11-03) with the option to extend for two additional one-year periods (Res. Number 0654-11); and extended on October 8, 2013 (Res. No. 0771-13); and

WHEREAS, the awarded contracts were for a two year period with the option to extend for two additional one-year periods with this being the second and final extension period with 28 providers; and

WHEREAS, the original contract allows for a 1.5% increase or decrease in rates with five (5) agencies choosing the increase and the remainder keeping their rates the same. These rates will be firm and fixed throughout the contract extension period and under the same terms and conditions as originally bid; and

WHEREAS, the term of the contract extension will be effective beginning October 1, 2014 through September 30, 2015; and

WHEREAS, the Franklin County Office on Aging is satisfied with the services provided by the contractors and recommends the contract extensions with the contract agencies listed herein be approved for the period of October 1, 2014 through September 30, 2015; now, therefore

BE IT RESOLVED BY THE BOARD OF COMMISSIONERS, FRANKLIN COUNTY, OHIO:

That the attached extension contracts are hereby approved for the following twenty eight (28) agencies as needed for services at the rates specified for the period of October 1, 2014 through September 30, 2015, and the attached PO# 41101000-00 has been opened to encumber funds for the remainder of the year:
