Resolution authorizing contracts with twenty-five (25) Contractors for Senior Options Medical Transportation (Lift and Non-Lift) Services (Office on Aging) (Estimated annual contract amount: $7,500,000.00)

WHEREAS, competitive bids were received and opened by the Purchasing Department on April 15, 2015 (ITB 2015-11-05); and

WHEREAS, the Franklin County Office on Aging administers Franklin County Senior Options which is a consumer-focused system of care which is funded by the Senior Services levy; and

WHEREAS, the expenditures for each type of service and the number of referrals made to each participating provider depends upon consumer need, consumer choice and service cost; and

WHEREAS, the term of the contract will be for a period of three years, beginning October 1, 2015 through September 30, 2018 and includes one optional two-year extension period; and

WHEREAS, the providers listed below submitted acceptable bids for Medical Transportation (Lift and Non-Lift) through Franklin County Purchasing Department pursuant to a competitive bidding process undertaken by the Franklin County Office on Aging, subject to the vendor agreements; and

BE IT RESOLVED BY THE BOARD OF COMMISSIONERS, FRANKLIN COUNTY, OHIO:

1. That the attached contracts are hereby approved for the following twenty five (25) provider agencies as needed at the rates specified for the period of October 1, 2015 through September 30, 2018, and the attached PO# 51101000-00 has been opened to encumber funds for the remainder of the year:
