Ambetter from Superior HealthPlan (Ambetter) makes it easier for you to get the care you deserve--from the doctor you trust. We partner with Primary Care Providers (PCPs), specialists and other medical providers.

In this book, you'll find a list of all of the providers in our network. If you choose to visit a provider that is not part of our network, your costs for receiving care will be affected. Remember that you might need a referral from your PCP in order to get care from certain providers or specialists.

When receiving care at an Ambetter participating hospital it is possible that some hospital-based providers (for example, anesthesiologists, radiologists, pathologists) may not be under contract with Ambetter as participating providers. These providers may bill you for the difference between Ambetter's allowed amount and the providers billed charge ? this is known as "balance billing". We encourage you to inquire about the providers who will be treating you before you begin your treatment, so you can understand their participation status with Ambetter.

Your health. Your choice.

Your PCP is the main doctor you will see for most of your medical care. This includes your check-ups, sick visits and other basic health needs. When you need care, all you have to do is call your PCP for an appointment.

Make sure to bring your Ambetter Member ID card when you receive medical care or fill prescriptions.

All Ambetter from Superior HealthPlan members will need to select a PCP. Use this directory, visit Ambetter. or call Member Services at 1-877-687-1196 (Relay Texas/TTY 1-800-735-2989).

We believe that having a good relationship with your PCP makes it easier for you to stay healthy. Be sure to choose someone you are comfortable with as your PCP. We recommend that everyone in your family have a PCP.

This directory also includes phone numbers for Ambetter's dental and vision services partners. They can help you find a participating Ambetter provider near you. If vision or dental is part of your plan, you will be covered for these services when visiting an in-network provider. If you did not buy a vision or dental plan, Ambetter will not cover visits to those providers.

Dedicated to your needs.

We understand that you might need some extra help. If you use a wheelchair, walker or another aid, let your PCP's office know when you make your appointment. Ambetter can always help you with talking to your doctor about your special needs. Just call Member Services at 1-877-687-1196 (Relay Texas/TTY 1-800-735-2989). We can help you access your PCP's office, and someone will be ready to help you when you get there.

Stay in charge.

Call your PCP if you have an illness or injury that needs attention right away. If your PCP isn't available, call our 24/7 Nurse Advice Line at 1-877-687-1196 (Relay Texas/TTY 1-800-735-2989).

For serious emergencies, call 911 or visit the Emergency Room (ER). Call your PCP or our 24/7 Nurse Advice Line if you're not sure if you should go to the ER.

Some examples of medical emergencies are: - Broken bones - Bleeding that won't stop - Labor and/or bleeding during pregnancy - Severe chest pain or heart attack - Drug overdose - Poisoning - Bad burns - Gun or knife wounds - Shock (symptoms: thirst, dizziness, pale skin) - Convulsions or seizures - Trouble breathing - Sudden inability to see, move or speak

You can use any hospital to receive emergency services.
