QUT prepaid card – terms and conditionsCONDITIONS OF USE OF THE QUT PREPAID CARDQUT Prepaid Card is a reloadable MasterCard branded card, provided in Australian Dollar currency onlyQUT Prepaid Card is treated as cash and can be used anywhere the MasterCard logo is acceptedQUT Prepaid Card has a card expiry - maximum period of 3 years, from issue date of the cardQUT Prepaid Card has EMV, Chip and PIN facilityMaximum withdrawal up to $400 per day from an ATM (Domestic ATM, fees apply and vary between $2 to $4 per transaction (No fees for using CBA ATM))Cannot be used in any CBA branchMaximum funds on the card in any month is $5,000Can be used at any International ATM, fees apply of $4 applies to transactionFees apply to overseas/international transactions – international foreign currency conversion plus 2.95% foreign currency transaction fee; international currency conversion plus 2% AUD currency transaction feeQUT Prepaid Card will decline if insufficient funds availableQUT Prepaid Card may decline if the ATM has insufficient funds in the machineDo not lend your QUT Prepaid Card to anyoneDo not provide your PIN or Password to anyone. You can access the card balances at any time by logging into the Cardholder Client PortalYou can print a card transaction report for the purchases undertaken up to 3 yearsYou are responsible for reporting card transactions to your research supervisors Do not throw your QUT Prepaid Card away if it is about to expire and you may be obtaining further funds or where there are funds remaining on the card but you have completed all activities.? In this circumstance, return the card to QUT Corporate Finance, Level 2, 88 Musk Avenue, Kelvin Grove, QLD 4059You can access and review your funds balance, change your PIN, Password and Security questions for the card via the Cardholder Client Portal websiteWHAT CAN’T I USE THE QUT PREPAID CARD FOR?QUT Prepaid Cards are not be used for any of the following activities:Purchases from merchants where the transaction is processed manually using an imprinter device – purchases using imprinter devices are not an acceptable purchase method. All cards must be used over the counter, online, or via an EFTPOS terminal Direct debits, recurring or instalment payments - payments established that are recurring in nature are not allowable using the QUT Prepaid Card. Do not enter into such arrangementsTransferring funds to other accounts – the QUT Prepaid Card cannot be used to pay or transfer monies from one bank account to another bank account. Cash withdrawal is available on the card.Any illegal activities – must not be used for any purposes, such as solicitation, drugs, gambling etc. STAFF ONLY:Purchase of Software, iPhones, iPads – QUT has various procurement policies for goods and services, such as software, iPhones and iPads. Cardholders should discuss with the Finance and Budget Officers from your Faculty / Division before undertaking such purchasesIN ADDITION TO THE ABOVE, THE FOLLOWING RESTRICTIONS APPLY TO STAFF AND LIKE STAFF (HDR STUDENTS)Goods and Services where a QUT Preferred Supplier Arrangement is in place Goods and Services where a QUT Standing Offer Arrangement is in placePurchases in excess of $5,000.00 (Excl. GST)Computer Software and Hardware - Asset purchases must comply with QUT’s procurement policyChemical purchases – refer to Chemical Management websiteFurnitureMedia AdvertisingMarketing ServicesMISUSE OR FRAUD ON THE QUT PREPAID CARDQUT or the CBA Bank may cancel a card if it is likely to be misused or suspected of illegal use. All cardholders are subject to the conditions and responsibilities under QUT’s Code of Conduct requirements as outlined in the Manual of Policy and Procedures, B/8.1 (Staff) and E/2.1 (Students). Cardholders will not have any sanctions placed on their accounts under this QUT Prepaid Card program. If an incidence of fraudulent use happens on your QUT Prepaid Card, it must be reported immediately to qut.fincard@qut.edu.au.The Manager, Accounts Payable and Receivable will refer all suspected fraudulent use incidences to the Director, Corporate Finance, who will report to the Registrar (Public Interest Disclosure Officer) as soon as practicable. The Registrar (Public Interest Disclosure Officer) will advise the Director, Corporate Finance of any further action - e.g. Potential Police action etc. Incidences of fraudulent use of QUTPC may result in the prosecution of the Cardholder, pursuant to Section 21 Financial and Performance Management Standard 2009.CARDHOLDER RESPONSIBILITIES FOR THE QUT PREPAID CARD Keep QUT Prepaid Card safe and in secure placeCreate their own PIN and PasswordDo not give prepaid card to another personDo not give PIN or passwords to another personDo not use card for illegal purposesKeep a check on your card balancesDo not go outside QUT Policies for procuring goods and services. Only use the card for official purpose intended as determined by QUTPrint a monthly statement of transactions for reporting and discussion with supervisorsKeep all Receipts for accountability and reporting purposesAdvise your responsibility area if the card is due to expire or to obtain additional fundsIdentify and report any fraudulent transactions immediately to the CBA on the Support Line 13 15 76If your card is Lost or Stolen:Lost/Stolen – Contact the CBA Immediately (13 15 76) to cancel the card and notify the QUT Corporate Card Officer (Corporate Finance) on qut.fincard@qut.edu.au. A new card will be issued by QUT.Where a card is stolen it must be reported to the Police. A Police Report number will be provided to the cardholder. Cardholder to provide the police report number to the QUT Corporate Card Officer (Corporate Finance) for follow up ................
