1. Wake Plastic Surgery's $5000 Makeover is open to female residents of the Research Triangle Area of North Carolina. Contestants must be 21 years of age or older. Employees of Wake Plastic Surgery, Cary Magazine (and its parent company S&A Cherokee) and their immediate families and the sponsors are not eligible. Wake Plastic Surgery's $5000 Makeover is a contest of Wake Plastic Surgery, with the contest promoted through Cary Magazine.

September 1, 2013 by completing an online entry form, writing an essay (500 words or less) on how Wake Plastic Surgery's $5000 Makeover will change their life, and submitting a recent photo of their face. Entry to Wake Plastic Surgery's $5000 Makeover will be via

entry per person will be accepted. Entries must be received via online submission by 11:59PM Monday, September 30, 2013. Each entry must be complete to be accepted. Neither Dr. William Stoeckel nor Cary Magazine assume any responsibility for entries not received. All materials submitted become property of Wake Plastic Surgery and will not be returned. 4. Dr. William Stoeckel retains the right to reject any entry form that he believes to be incomplete, illegible or inappropriate for any reason. The applicant must be in excellent mental and physical health and cannot be pregnant, or planning on becoming pregnant in the next six months. Aesthetic services and procedures are not suitable for everyone. A Wake Plastic Surgery medical aesthetician and injection RN will perform the non-surgical aesthetic treatments. Treatments may only be performed at Wake Plastic Surgery on a mutually agreeable date prior to December 31, 2013. 5. All eligible entries will be mandatorily screened by Dr. William Stoeckel and/or his representatives at Wake Plastic Surgery located at 300 Keisler Drive, Suite 102, Cary, NC 27518 in order to determine contest eligibility and the level of non-surgical aesthetic procedures that are necessary. Dr. William Stoeckel has the right to remove eligibility of any entrant based on findings during the screening process. 6. Contest judges reserve the right to decide all features of the awards offered while winning applicant has the right to refuse any or all of the procedures presented. All procedures are safe and any incidences of complications will be discussed prior to services. 7. The winner will be announced from entries received on or around October 7, 2013. Winner will be notified by phone and/or email no later than October 4, 2013 of their selection and must complete a personal interview to determine eligibility and will also be required to sign Wake Plastic Surgery's affidavit of eligibility in order to collect the prize. This affidavit must be returned within one week of date of notification or an alternate winner may be selected. If the winner is determined non-eligible, then an alternate participant will be chosen from the same application process and will be declared Wake Plastic Surgery's $5000 Makeover winner. 8. By participating in Wake Plastic Surgery's $5000 Makeover, entrants agree to hold Sponsors and Cary Magazine, their respective directors, officers, employees and assigns harmless against any and all claims and liability arising out of the award or use of the prize. 9. Prize: One winner of Wake Plastic Surgery's $5000 Makeover will receive a non-surgical aesthetic makeover from Wake Plastic Surgery. The maximum value of the makeover will be $5,000. There is no cash value to the prize and prize is non-transferable and non-assignable. Its value cannot be applied against previously performed treatments or surgeries. The winner will be issued an IRS form 1099 for tax purposes. 10. All entrants agree that sponsors may use her name, photographs, videos, contest entry and statements concerning Wake Plastic Surgery's $5000 Makeover entry, without compensation, for purposes of advertising and promotion in Cary Magazine and by Wake Plastic Surgery and also grants all rights to edit or modify and to publish and copyright it. By participating in Wake Plastic Surgery's $5000 Makeover, entrants agree to hold sponsors and Cary Magazine, their respective directors and employees harmless against any and all claims and liability arising out of the award or use of the prize. 11. Winner must be available for several days for the non-surgical aesthetic makeover process and must make all appointments. Missing appointments could jeopardize the completion of the makeover and could result in disqualification. If the chosen participant is disqualified for any reason, an alternate participant will be chosen to complete Wake Plastic Surgery's $5000 Makeover. The alternate will be chosen from the same application process. 12. All questions of compliance, disqualification or other contest rules will be answered and determined by and at the sole discretion of Dr. William Stoeckel. 13. Any questions or concerns about rules must be brought up at the time that they occur. No issues will be considered after the contest ends.

14. All federal, state and local laws and regulations apply. Void where prohibited or restricted by law. Taxes, if applicable, are the winner's responsibility. All information submitted by entrants as part of this Contest may be used by Sponsor to communicate with entrant for Sponsor's or third party marketing purposes. No purchase necessary.
