Credentialing

License has been expired for over 5yrs. Licensee must apply to reinstate the

license. Refer to website for forms or to Initial Credentialing workgroup.

Additional Funds Needed. This only shows up if the license renewal fee wasn¡¯t

paid in full.

Agency Affiliation Required. This is used for Security professions. They must be

employed by a licensed Agency.

Renewal Application. This is used when special forms are required for renewal.

Refer to renewal requirements on website.

Basic Credential Required.

Bureau Hold. Check History for reasons of hold.

Criminal Information Background.

Certification of Legal Status.

CPR/AED certification must be up to date to renew.

Evaluation and Protocol form for Athletic Trainers.

Name and Address Change Requested On-line.

If a RN holds a Nurse Midwife (NMW) license, no fee is required, if the nurse

renews the NMW license first.

National Certification date MUST have an expiration date that occurs AFTER

renewal date.

Pending Charges and Convictions.

Private Detective Agency affiliation was either left blank, affiliation has ended,

or agency¡¯s license is expired.

Proof of Continuing education is required for Appraisers who are renewing

late.

Signature attestation required. Refer to professional renewal requirements on

website for description.

Social Security Number. Licensee either reported the last 4 of their SSN/FEIN

online incorrectly or SSN is missing.

Status of License. Only for internal use by renewal unit. No action needed by

licensee.

DWD Nurse Workforce Survey required for renewal. Waived if returning from

compact state.

Transfer of Employment Form and $10 fee required (Real Estate Brokers).

Transition of License requested from Cosmetology to Barbering. No longer

used.

Work History Description required for Athletic Trainers.

Withdrawal of Renewal Application.
