Authorization and Release, Liability WaiverGeneral Terms: The Doggy Den will exercise reasonable care and safety of your pet, and keep the boarding premises safe and clean. The Doggy Den cannot guarantee against accidents, and we cannot be liable for loss or damage caused by or to pet guests at this facility. You hereby agree to release from liability and hold harmless The Doggy Den, its owners, its employees, contractors, or anyone else associated with The Doggy Den (collectively called “Releasees”) from any and all liability claims, litigation, or suits regarding any act or omission of the Releasees or any injuries to your pet while in our care. You also agree that you are solely responsible for any and all attacks, expenses, or damage caused by your dog while it is at this facility. Vaccinations/Flea & Tic Prevention: All guests must be vaccinated for rabies, kennel cough, and parvo/distemper. Vaccinationsare for the protection of your dog, and we cannot make exceptions to vaccination requirements. If your dog’s vaccinations expire between visits, they will need to be revaccinated/boostered and wait for the required time for them to reach full effectiveness per accepted veterinarian protocol. Once a vaccine is administered, the antigens must be recognized, responded to and remembered by the immune system. In most animals, disease protection does not begin until five days after vaccination. Full protection from a vaccine usually takes up to fourteen days.There is also the possibility of illnesses being transmitted from dog to dog or human to dog. While all guests are required to be current on the core vaccines, some illnesses or other physical conditions/ailments may not be apparent to you or The Doggy Den at the time of check-in and only manifest later in his/her stay. Examples may include but not be limited to being in the incubation period of kennel cough but not yet symptomatic, or experiencing an unforeseeable reaction to prescribed medication. Situations such as these are beyond our control. We recommend your dog(s) be kept current with flea/tick preventative. If we discover fleas/ticks on your dog while he/she is in daycare or boarding we will automatically give them a flea bath and call you for immediate pick-up. If we discover fleas/ticks on your dog while he/she is being groomed we will automatically administer a flea bath/baths as part of the groom. We will require current proof of flea/tick treatment and will administer a visual inspection of your dog for fleas/ticks at drop-off on the next visit. If fleas/ticks are found on your dog, your dog will not be allowed to stay. You will be charged for any flea bath/baths we deem necessary whether you have been notified or not. By signing this document, you agree to release The Doggy Den from any and all responsibility and /or liability for any illness, sickness, reactions, bloat, or any other thing that results in injury, sickness or death while in our care. Departure Baths: We offer complimentary departure baths for all dogs staying 5 or more consecutive days. If you do NOT want your dog to receive the departure bath, please notify us at check-in. If your pet is here less than 5 days and you would like to purchase a basic departure bath, the cost is $25.00. Personal Items: The Doggy Den recommends against bringing personal items from home, as they may cause, not reduce, separation anxiety for your pet. The Doggy Den will not be responsible for lost/damaged personal items. We do recommend bringing enough of his/her own food for the duration of their stay. Sometimes dogs will refuse to eat due to stress or other factors. We may need to add healthy “wet” food or offer some other brand of healthy food to entice them to eat. You herby authorize us to what we deem best to ensure your dog gets proper nutrition while in our care. If you have indicated on The First Time Guest Registration Form that your dog is allergic to certain foods or has other concerns regarding what he/she eats we will NOT feed them any food but their own until we receive permission from you or the person authorized to make that decision. _____ Initial Spay/Neuter: Guests must be spayed/neutered at or before 9 months of age. We reserve the right to require individual dogs to be spayed/neutered earlier than 9 months at our discretion. Medication and Treatment Authorization: The owner agrees that The Doggy Den may, at its discretion, give first aid, medication, or other attention if we deem necessary for the safety and health of your dog. If your dog is allergic to any common medication (example-Benadryl) that should be disclosed in the “medical history/information” section on this form. The Doggy Den is authorized by the owner to seek veterinary advice and care, including emergency care, at the owner’s expense. If we believe that your dog is in need of veterinary care, if time permits, we will try to contact you before obtaining that care, but this document serves as our authorization to obtain veterinary care for your dog regardless. The owner is responsible for expenses of veterinary care and transportation, whether or not you have been reached in advance. By signing this authorization, you give The Doggy Den the authority to make reasonable care decisions regarding your dog, and agree to pay for all costs incurred for such treatment. This authorization applies to dogs at The Doggy Den for any purpose, including but not limited to, Daycare, Boarding, and Grooming. _____ InitialDaycare/Group Play Authorization: Daycare and group play necessarily involve dogs interacting and playing together. Dogs in daycare may play in the pool, toss toys, and romp with other canine guests. Although these activities are supervised, anytime dogs play together there is the possibility of injury from rough play or combative behavior. Dogs in daycare or group play may be outside for extended periods of time and may get wet. We reserve the right to refuse any dog at our discretion that we are uncomfortable with for any reason (examples include but are not limited to; poor health, advanced age, socialization issues, aggressiveness) We also reserve the right to remove dogs from group play that we feel are posing a threat to the safety and /or stability of the group. Dogs that, in our judgment, can’t assimilate successfully will not be allowed to return for Daycare or Boarding. All guests must be at least 5 months old (20 weeks). We require that your dog has an introductory/evaluation Daycare day in which he/she spends at least 5 hours with the group prior to being accepted for regular daycare or being booked for any boarding stay. The first time your dog visits us is complimentary, if they haven’t been in group play at our facility in the last 5 months they will be required to have a re-evaluation visit at the client’s expense before being scheduled for a Boarding stay. Please allow my dog to participate in Daycare. Yes ______ No ______Your signature at the bottom of this document acknowledges that you are aware of the risks involved with any canine group activities, and that you agree to hold The Doggy Den harmless for the associated risks and are authorizing your dog to enter into our group play or daycare program.Payment for services: The Doggy Den charges for boarding by the day. (24-hour period) Full payment is due upon check out. If on the day your dog goes home, he/she is picked up after being here for 24 hours, either the half-day or full-day price for daycare will be added to the price of the stay depending on what time the dog is picked up. Example: A 3-night stay=$105.00 + $12.00 (half day-5 hours) or $22.00 (full day-more than 5 hours). Payments for daycare packages are due in advance for the week/month attending. The five (5) day pass can be used over a one-month (30 day) period. The ten (10) day pass can be used over a two-month (60 day) period. The twenty (20) day pass and can be used over a three-month (90 day) period. Any unused days will expire at the end of the given time frame. These packages are for daycare, and no other services may be deducted from daycare packages. Half days (5 hours) must be paid for separately.Abandonment: If your dog(s) is still at The Doggy Den seven (7) days after the scheduled pick-up time with no contact from you or your representatives/emergency contact(s), it will be deemed “abandoned”. This means you forfeit all ownership rights and The Doggy Den becomes its legal owner. The dog(s) will become the property of The Doggy Den and we will do with it/them as we see fit. You understand and agree that you will permanently give up all claim to the dog(s), and by signing this document waive all rights to pursue any action, legal or otherwise, to reclaim your dog after it is abandoned. This agreement will remain in force for all my pet’s visits to The Doggy Den, and for any services including but not limited to Daycare, Boarding, and Grooming. I hereby agree to the foregoing conditions as the owner of the aforementioned dog(s) listed on The First Time Guest Registration Form. I further certify that my dog(s) is (are) in good health and have not been ill with any communicable condition, nor to my knowledge been exposed to any communicable diseases within the last 30 days. Moreover, I certify the accuracy of all information given about my dog(s) and that my dog(s) has not harmed or shown aggressive or threatening behavior toward any person or animal. I assert that the information I have given is complete and accurate to the best of my knowledge. I agree that this entire agreement is binding on me and any/all of my representatives, including but not limited to, my emergency contact(s). I have read, understand, and agree to the foregoing.
